Abstract
We report a result of the nourishment care management. A factor of middle and high risk are low BMI and low albumin. Those ratios are 86% in total. Thus, we introduced a protein supplement. In addition, we polished important documents for communalization of information. We show these templates, nourishment screening book, nourishment assessment book, a nourishment care plan, a conference simplified form, nour-ishment care monitoring, a nourishment care offer progress simplified form.
