We report a case of traumatic rectal penetration by a hand gripper that was inserted for masturbation and that was removed by laparotomy. A 39-year-old man often inserted the hand gripper into his anus during masturbation. After one such incident, he could not remove it. About 20 days later, buttock pain appeared and he visited our hospital. We diagnosed traumatic rectal penetration by a hand gripper, and performed an emergency laparotomy because it could not be removed transanally. He was discharged 10 days after the operation without any complications. To the best of our knowledge, this is the first report in the Japanese literature of a case of a transanal rectal foreign body (hand gripper).
