The case was Angle class I malocclusion with crowding and maxillary and mandibular protorusion. A decision was made to extract two upper 1st. bicuspids and two lower 2nd. bicuspids and to use a palatal bar for maxillary anchorege.
The case and diagnosis was popular in lingual orthodontic treatment of Japanese patients.
