The clinical investigations on the malar bone fractures treated in the past 5 years are reported with a proposal of the direct operative method for the arch fractures.
Out of 31 malar fracture instances, 25 arches were involved. Persisting jaw trismus, however, was sometimes noticed immediately after the injury was observed in only a few cases. In 15 cases of the arch fractures complicated with the clear deformity, persistant jaw trismus or other fractures, the direct approach method was applied without sequence of facial nerve. Some merits of this surgical technique such as operative easiness, exact reduction and no requirement of the fixing equipment are advocated in view of the good results of the experience.
