Treatment Using Adsorption-Type Blood Purifier Rheocarna for Refractory Chronic Limb-Threatening Ischemia

Abstract

 Introduction: Chronic limb-threatening ischemia (CLTI) is a severe manifestation of lower extremity artery disease. Its prevalence is increasing due to aging populations and the increasing incidence of diabetes. This study assessed the effectiveness of the adsorptive blood purification device "Rheocarna" for the treatment of CLTI for which revascularization procedures were not feasible.
 Methods: We retrospectively analyzed treatment outcomes and changes in the inflammatory marker C-reactive protein (CRP) in 15 patients who were treated using Rheocarna at our hospital between 2021 and 2023.
 Results: Healing or reduction of ulcers was observed in 10 cases (67%). Some cases showed decreased CRP levels. There was no significant difference in treatment outcomes when Rheocarna was combined with hyperbaric oxygen therapy.
 Discussion: Rheocarna may help improve peripheral blood circulation, promote ulcer healing, and reduce inflammation.
 Conclusion: Rheocarna may have therapeutic efficacy in cases of CLTI in which revascularization was difficult. Further studies with a larger cohort are necessary to determine which patients benefit the most from this treatment.
