A 40-year-old woman was admitted complaining of dyspnea on exertion and cough. Chest X-ray films showed bilateral diffuse small nodular shadows. Total cell count and the percentage of lymphocytes increased. The CD4/8 ratio in bronchoalveolar lavage fluid decreased. TBLB specimen showed non-caseous granuloma and infiltration of lymphocytes in the alveolar wall. Anti-Aspergillus antibodies were detected in the serum. She worked on an orchid farm and changed orchid culture medium in which Aspergillus fumigatus was detected. We diagnosed this case as occupational hypersensitivity pneumonitis caused by aspergillus fumigatus.
