A 54-year-old woman presented with complaints of repeated episodes of pyrexia and productive cough. Chest X ray and CT films showed an endobronchial tumor in the right upper lobe bronchus, and obstructive pneumonia of the right upper lobe. Bronchoscopy revealed a smooth-surfaced tumor protruding into the right upper lobe bronchus. Bronchoscopic biopsy suggested an endobronchial leiomyoma. Right upper lobectomy was performed because of recurrent obstructive pneumonia. The resected specimen contained a smooth-surfaced tumor, 10 mm in size, protruding into the right upper bronchial lumen with a base attached to the right B^2 bronchus. The specimen showed organizing pneumonia with necrosis in the right S^3 region. Primary benign tumors of the lung are relatively rare and leiomyoma of the lung is particularly rare. We report a case of bronchial leiomyoma with recurrent obstructive pneumonia.
