A Case of Endobronchial Hamartoma with Bronchiectasis, Mimicking Lung Cancer

Abstract

Background. Endobronchial hamartoma is rare compared with intrapulmonary hamartoma. We report a case of endobronchial hamartoma presenting as a mass lesion leading to bronchiectasis and obstructive pneumonia. Case. A 54-year-old man was referred to our hospital because of a mass lesion on a chest X-ray film obtained on a routine health check. Computed tomography showed a mass lesion at the right lower lobe obstructing the orifice of right B^8. We performed transbronchial biopsy but obtained no malignant findings. The tumor had a smooth surface suggesting a benign lesion, and the area distal to the mass showed no abnormalities. PET-CT suggested a high possibility of malignant tumor. Considering the difficulty to remove the tumor with an endobronchial snare, a right lower lobectomy was conducted. Histopathologic findings showed an endobronchial hamartoma in 15 mm diameter at the right entrance of B^8. Bronchiectasis and obstructive pneumonia were shown at the distal portion of the nodule. Conclusion. A hamartoma at the entrance of right B^8, caused bronchiectasis and obstructive pneumonia and, as a result, developed a mass in the lung.
