Abstract

In an era of low growth, it is an important subject for Japan to switch over the medical system from hospital care to home healthcare. In this paper, focusing the core hospitals, we analyzed the flow of leaving patients from hospitals, by using the leaving patient surveys conducted in Toyota, Aichi, and researched the current situation of home healthcare. As a result, we emphasized that it is important to ascertain where the bottleneck to enhance the number of home healthcare patients is, monitoring the flow of leaving patients from hospitals. This flow could be the management index considered so as to proceed to develop an integrated community care system. After that, all concerned should implement effective measures to eliminate the bottleneck together.
