We experienced a new procedure of the decompression of thoracic OPLL. This procedure is as follows. The vertebral body and rib were exposed by extrapleural approach. First a proximal side of the rib was resected by air drill. Secondly the pedicle was resected. At this time dural tube was observed, and anterior decompression was performed safely.
