We experienced four cases of lateral disc herniation between L4 and L5.
All cases complained of severe pain in the lower extremity accompanied by disturbance in walking.
All patients were evaluated with myelography, magnetic resonance imaging (MRI), discography, computed tomographic discography (CTD) and selective radiculography and block. Accurate diagnosis of intraforaminal or extraforaminal herniation was made with axial views of MRI and CTD. Selective root block was useful for detecting the level of the root lesion.
We performed lateral fenestration and removed the disc herniation in all cases.
The surgical outcomes were excellent in all cases.
