We describe a new technique of internal fixation that was used for two supracondylar fractures of the femur proximal to a total knee arthroplasty. We report our experience in the mamagement of two patients with this problem and some thoughts on its etiology. Two patients who had low supracondylar fractures above total knee arthroplasties were treated using the intramedullary supracondlar nail. One of these fractures may be associated with a surgical notching of the anterior femoral cortex during resection of the patellar trochlea. Two fractures healed while preserving nearly all of the prefracture range of motion. The nail provides stable internal fixation with good control of rotation and allows early range of motion.
