We report our experience using the modified transgluteal approach to the hip which is described in the literature.
Between 1994 and 1996, we examined 43 people (48 hips), 13men and 30 women, with a mean age 59 years (range, 18 to 83 years).
The approach is exposure of the hip by osteotomy of the anterior part of the greater trochanter and the whole of the gluteus medius and minimus and the vastus lateralis are attached to the osteotomised fragment. We studied the evidence of this approach.
Continity was preserved along the gluteus medius and minimus and the vastus lateralis in all cases and the power of the abductor muscles were preserved. Complications included displacement of the osteotomised fragment and heterotopic calcification.
