Abstract

Responses of serum gastrin and gastric acid output to a constant intravenous infusion of secretin were studied in 37 cases of peptic ulcer and a case of Zollinger-Ellison syndrome (ZES). The responses were closely related to the activity of the ulcer. Serum gastrin increased in the active stage and decreased in the scar stage, in response to the stimulation. Although either Boots secretin or Eisai secretin and or GIH secretin was infused in some cases, these different responses were observed in the same individual. Gastric acid output was significantly suppressed by secretin infusion only in the scar stage. There was no difference in the attitude between gastric and duodenal ulcers. Our results may indicate that a paradoxical response of serum gastrin to the secretin infusion is not necessarily specific to ZES. In a case of ZES, serum gastrin response to secretin infusion markedly increased in the active stage, but slightly increased in the scar stage.
