Intussusceptions in adults are rare. We report a resected case of malignant ileal lymphoma presented with intussusception in an adult.
The case was a 26-year-old male who had been suffering from epigastric pain since 5 months before. On admission, the patient complained of nausea, and abdominal simple X-ray film revealed a gaseous figure in the small intestine. A infant's fist-sized tumor was also palpated in the right flank. The tumor manifested as concentric circular findings by CT and ultrasonography. Barium enema and endoscopy of the colon disclosed intussusception in which the ileal tumor invaginated as the head. On laparotomy a 3.0×3.0×2.5cm tumor was found in the ileum 22cm to oral side from Bauhin valve, and double intussusception in the ileum-ileum led by the tumor and ileumcolon was observed. Regional lymph nodes showed positive metastasis and the patient received right hemiwlectomy, followed by chemotherapy. Histologically it was a diffuse and large cell-type malignant lymphoma. No recurrence is found as of 4 years and 8 months after surgery.
