We report an extremely rare case of early gastric cancer associated with an aberrant pancreas of the stomach. A 70-year-old woman was admitted to the hospital for close examination, because she was pointed out an abnormal shadow of the stomach at a mass screening for gastric cancer. Upper gastrointestinal Barium study and endoscopy revealed a submucosal tumor (SMT) 1.5cm in diameter at the greater curvature in the antrum. Endoscopic ultrasonograpy suggested this SMT as an aberrant pancreas in the stomach because of its echogenecity. During a follow-up period for 8 months, an irregular depression was revealed on the mucosa of the SMT endoscopically and endoscopic biopsy demonstated Group V. Distal gastrectomy was performed. Postoperative histopathological findings showed that well differentiated adenocarcinoma was limited within mucosa at the top of an aberrant pancreas (Heinrich II Type). No continuity between the gastric carcinoma and aberrant pancreas was observed histologically. So far there has been no case report of carcinoma arising in just above the gastric mucosa where an aberrant pancreas like this case.
