A case of 53 years old woman with foam cells in bone marrow and spleen was reported.
She was admitted because of fever and left upper abdominal pain. Hepatosplenomegaly was prominent. Laboratory studies revealed moderate leucocytosis and thrombocytosis. Lymphocytes had large vacuoles. Alkaline phosphatase activity of granulocytes was not decreased. The value of total lipid, phospholipid and cholesterol in serum were moderately elevated. The ultrastructure of lymphocytes vacuoles suggested lipid accummulation.
The bone marrow picture was hyperplastic and there were many foam cells which showed strong activity of acid phosphatase. There were many phagosomes in foam cells on electron microscopic observation.
She was treated with steroid, antibiotics and busulfan. Eight months later after admission, she died of dyspnea.
The autopsy revealed that there were many foam cells in splenic sinuses.
