    The attempt to spend comfortable hospitalization life and to feel life rhythms during the stay. We made the awareness check sheet, and planed the recreations based on the records through the broadcasting into all the rooms of our A3 ward, change of clothes in the morning or at night, and recreations. As a result, patients became energetic, improved their organs' functions by moving body pleasantly, and refreshed their mind.

  • Our meta-communication experiences

    Ministry of Health, Labor and Welfare Japan showed that there were 6646 ALS (amytrophic lateral sclerosis) patients. These diseases patients are increasing year by year. Here we described that how we commu-nicated and the meta communication might be the best way to contact different levels of ALS patient. The re-sults indicated that meta communication stabilized the patient emotion, and release her from anxiety. Especially the number of nurse call decreased from 100 times/day to 10 times/day. She complained about muscle weak-ness in arms or legs, a hoarse voice, difficulty walking, clumsy hand, exercise intolerance, abnormal fatigue, and her life should be vanished. We planed to take a wheelchair riding to see beautiful sight, and we introduced her smile and satisfy.

    HDS-R scores was used for Training plan to improve vascular dementia. "Kanahiroi" test was em-ployed to diagnose frontal brain function by circling [A], [I],[U],[E], and [O] characters appeared in the spe-cific story described Kanas. The other mental test and activity of daily life also used for the judgment of de-mentia levels. Memorization training using painted 10 vegetable cards each had order number to show on them were prepared 10 ways to show-sets and trained to memorize and assessed. We also planed and trained to cal-culate mentally, to memorize numbers and say reversibly, and to calculate by writing. These trials made possi-ble to assess dementia level precisely by introducing NM and N-ADL scales, to find out early dementia symp-toms, and to plan schedule to improve dementia level.

    Our daycare team succeeded in aged patients to be more healthy with daily wheelchair exercises. We show in this paper that we encouraged the aged patients to attend 5 plan trainings such as 1,walking on sla-loms[sec] 2,up and down steps[times] 3,strength of grip(left)[kg] 4,strength of grip(right)[kg] 5,lifting dumb-bells[times]. We showed them their segmented training data as the first, the middle and the final period. As a result, they joined in training time positively and built up their physical ability like walking, grip and lifting dumbbells. During muscle strengthen training they felt like recreation and it caused mental effect induced smile and deligh speech communication each other.

    We introduced not only drug uptake and teaching but also report system for the patient who have natural right to receive excellent quality of medication. Working manual had established by introducing POS (Problem Oriented System) and SOAP. The pharmaceutical SOAP note approach had performed. SOAP is an acronym for Subjective data, Objective data, Assessment and Plan Those created according to the information sheets on the patient history of the prepared medicines in the pharmacy. Considering POS & SOAP, we made severe side effect checklist and predicate side effect checklist those used both at economically limited and unlimited wards. Effective working guidance and standardization were performed where taking medicine and teaching drug effects and side effects. These efforts lead us the methods change from quantity to quality.

    Indication of entral nutrition is limited to diseases of gastro intestinal tracts, diseases require intensive care, and swallowing disorder. These medicines were dripped into gastro intestinal tract by tubing. From the opening of Showa Hospital, we gave patients their medicines after powdered or suspended into hot water. However 4 problems we had to solve, 1: powders mixed obstructed the tube, 2: frequently aspirate or paste on hands the mixed powders, 3: quantity loss occurred, and 4: extra mixing time required. Nomi Kurata's simpli-fied suspension protocol was to sink the tablets or capsules into 55C hot water immediate before swallowing. This method was very useful and solved every 4 problems.

    A microwave oven, a hand phone, a personal computer, a television etc. make our lives very com-fortable. These machines, however, irradiated us electromagnetic waves unconsciously. Recently, several re-search reports indicated that electromagnetic wave caused suppression of melatonin production and occurrence of a brain tumor. WHO and Japanese Ministry of Health, Labor and Welfare developed an electric wave pro-tection indicator. Under these circumstances, we surveyed the recognized levels of those electromagnetic waves, and also measured quantity of electromagnetic wave from each personal electric appliance. The sur-veyed data indicated that 72% people anxious of it but only 14% protected it. No measured values reached the WHO standard, for example, hot carpet: 205(+/-)15, micro wave oven: 70(+/-)10, medical CT cave: 69(+/-)59mg etc. In general asbestos induces malignant pleural mesothelioma which used to recognized as useful protector from fire, and ultra sonic recognized as harmless while induce some effect to the infants. The knowledge of electromagnetic wave might save our life.

    Swallow difficulty patients were generally supplied swallow training meals or fractured meals. However, swallow training meals and fractured meals are hard to eat because of bad appearance and muddy form. We described here, how tried to change swallow training meals and fracture meals into soft meal to eat easily. Trial manufactured meals divided into two types, one was added agaropectin alone and the other was super added gelatint. We took questionnaires which one was better feeling of appetites, properly arranged, less separated from water, less sticky, less gritty and more delicious. We found that the patients could swallow the meals with added gelatin more smoothly without feeling of appetites. Further more, we tried to change other foods into soft meals. We felt delicious about soften salad and pressed vegetable wore dressing before eat, be-cause the exudates suppressed appetite. We served emulsified dressing with increased quantity because its tasted lighter than non-oil dressing and soy souse. Boiled food was difficult to change to soft meal.

    Nutrition Support Team (NST) was introduced to improve nutritious status to cause eating orally, and to protect infection by strengthen immune systems to heal pressure ulcer. Case 1: 92 years' male K, cerebral infarction, and swollen disturbance. Methods a) Nutrition state was determined by Subjective global assesment protocol. b) Swollen state was determined by Video fluorography. c) Panel describing "Eating-difficulty care attentions" was hanged to make sure among stuffs. d) To eat one jelly/day, e) NST round (assessed each pa-tients once a week); Case 2: 77 years' ffemale T, pleasure ulcer at sacrum, disuse syndrome, Methods a) & e) the same as K. b) estimate a pressure ulcer/week, c) measure body weight, over arm circuit, subcutaneous fat thickness and biochemical blood assay/month, d) use the protein, fat, and water data to calculate calorie to change the eating menu. Results: Mr. K had swollen disturbance, however talked with a smile after become possible to eat 1 to 2 jellies/day. Mrs. T showed low nutritious state as 27.2 kg of bodyweight, TP 5.1, Alb 1.9 (Stage III pressure ulcer at sacrum), then we gave essential macrominerals, V CRESC and IMPACT for im-mune- strengthen, that lead her bodyweight increase about 12.2 kg (TP 7.2, Alb 3.5), then she could ride wheel-chair by herself.

    We started BSC (Balanced Score Cards) project team with 4 points of views. 1: Economical view, 2: Patients' view, 3: Hospitalization view, and 4: learn and developmental views. After application of BSC, mem-bers recognized that necessity to know the legitimate expense control, to construct management system for calculate the expense of each patient, and to use all beds effectively. Rehabilitation standards made up to total rehabilitation system, and the employment of a dentist led the increase of outpatients. As for the results, the economical income increased that generated to make all stuff's efforts.

    We improved the childcare strategy for disabled child nursing during various children's growing up. The observation practices, through the volunteer work at recuperative facilities for the handicapped, improved the care technology and reconsider the attitude to the deformity children. Moreover, the meeting was held for each child to reconsider care plans. Consequently, when the stuff finished practical work, they could accept deformity children with confidence. Moreover, the common understanding among stuff led the individual childcare plan more suitably and unified the planning protocol.

    Elderly care insurance system had passed 6 years that started at 2000. Insurance system is going to change to low cost and unprofitable version form April 2006. The 70% of customers have to shift from nursing care insurance to new preventive care system from disease. Then nowadays-home care support business has to manage serious and middle stage patients. We examined our business to restore from this big revolution. Our center had been made great effort the pre-stage or stage I do not enter stage II or stage III care levels. In case pre-stage, we succeeded 50.9% had maintained the same pre-stage care level, and suppressed until 28.4% making worse from this pre-stage care. On the other hand, 17.9% improved the care level 1 to pre-stage care. These scores were much better than all Japan average care. These data indicated that our nursing care and ser-vices were high quality. We tried home nursing care to the terminal Parkinsonism for coming new system. We succeeded without any trouble at home care for 3 days that included visit care 2 or 3 times in a day and visit nursing once in a day.

    Sakurayama home-helper station started to support mental patients and children's developmental disorder from May2005 after nursing care insurance changed. We needed to study about these cases for future study. This report includes three cases we experienced. Case 1 was approach to a girl's pervasive development disorder. We reduced her stress and gave her anxiety that caused her to stop injuring herself. Case 2 was a woman's recurrent depressive disorder. We got rid of her anxious and gave her more confidence by attending the social community. Case 3 was a living alone with total blind man suffered from diabetic retinopathy. At first we tried to get his trust in us cautioning his dangers by voice talking, that caused more reliable feeling between us and made his mind stable. Finally he understood home-helpers and he hardly showed feigned mental illness pretending hypoglycemia.

    Power rehabilitation was introduced at Phoenix Facilities for Day Services from January 2005. Pro-gram was repeated twice daily at 10:00 and 13:00. The trainings prolonged for 2 hrs, which consisted with pre-gymnastics, power rehabilitation for the main and coffee brake. August 2005, the effect of power rehabilita-tion surveyed questionnaire and found mental outcome was significant. The body functional assesment indi-cated that 52.6% male and 75% female felt lighter body weight. As for the spiritual elevation, 87.5% of female replied, "I might feel easy daily life". About 50% female and 42.1% male customer of the power rehabilitation replied, "Activation of daily life was brought up after receiving power rehabilitation".

    This paper deals with a navigation control and a remote of an electromotion wheelchair in a hospital site. This navigation system has two computers, a host machine set on the wheelchair and a remote machine set on the control center (or a nurse center). The wheelchairs carry the patients and the handicapped persons. Though it is a very useful vehicle, a nurse or a supplementary person have to push the wheelchair. Therefore, the system, that is able to lead the wheelchair to the destination area (goal) by itself without needing any sup-plements, is expected. In this paper, the recognition of an environment structure, a route search of a goal to move the wheelchair, and the designs of the fuzzy rules to realize the wheelchair navigation are done. After that, the fuzzy navigation control system is developed. The navigation system is consists of a predictive avoid-ance control unit, the route guidance control unit (based on a site map and self-location of wheelchair), the re-mote control unit (based on no map or the lost state of self-location), and so on. By this navigation system, the moving environment for the wheelchair with a little load can be realized. In addition, the patient privacy is also protected.

    We have developed the novel Fuzzy Set Chaos Algorithm (FSCA) to regulate tissue histology. If serum quantities of molecules that activate the expression of genes and proteins are at fuzzy levels, then histologi-cal regulation could be accomplished by iterative ON/OFF expression programming cycles and the resulting plot-pattern would render a depiction of chaos. We tested this hypothesis on metastasized cancer cells that pro-liferate rapidly and migrate into adjacent tissues and organs. Using a basic flow chart of histological regulation, we searched various databases to identify effective molecules. These molecules were then used for gene expres-sion programming to envelop metastasized tumor cells by commanding fibroblasts to surround the target cells in the tissue. The ON commands we used were 1) switch on transcription of collagen genes by ascorbic acid (VC), 2) increase collagen mRNA translation by supplying collagen-enriched food or by amino acids Pro Gly and Cys, 3) envelop metastasized cancer cells by combining collagen with elastin which is activated by calcium, and 4) proliferate fibroblastic cells by thiamin (VB1), riboflavin (VB2), pyridoxine (VB6), and cobalamin (VB12) dur-ing the fibrogenesis. Elimination of the supply of individual molecules constituted the OFF command. The com-mands were transmitted via the blood flow; their delivery to the area of interest was possible because permeability of the walls of blood vessels around metastasized cancer cells is increased by IL-1, IL-6, TNF-a, PAF, prostacy-clin bradykinin and VEGF. Cytokines and chemokines around metastasized cancer cells served to inform fibro-blasts of the location of metastasized cancer cells. Successful of fibrogenesis was conformed by rat liver fibrosis and the effectiveness of the therapy by patient viability.
