Clinical significance of the several allergologic tests as a detective method of causal agents in asthma were studied. The direct intracutaneous skin test, the Prausnitz-Kustner reaction, the eye test, the inhalative provocation test (I. P. T. ), in vitro thrombo- and leucopenic indexes and the hemagglutination test with Middlebrook's and B. D. B. method were carried out simultaneously in 23 cases of asthmatic patients in the symptomfree interval by means of the house dust extract from one same lot. 1) The incidence of positive test were as follows: Intracutaneous skin test 61% P. K. Reaction 52% I. P. T. 26% Eye test 13% In vitro Thrombopenic index 25% or more 31% In vitro Leucopenic index 20% or more 17% Middlebrook's test 17% B. D. B. test 67% 2) The intracutaneous skin test was considered to be useful as a screening test for the detection of causal agents in asthma since all the patients with one or more positive test out of four test, i. e. I. P. T. , eye test, P. K. reaction and thrombopenic phenomenon reacted also positive to intracutaneous test. A high correlation coefficient was obtained between the skin test and P. K. reaction. 3) The results of eye test showed a close relationship to those of I. P. T. , and all of the 3 patients with positive eye test reacted positive to I. P. T. , P. K. reaction, and to thrombopenic phenomenon. However, there were three cases in which the eye test was negative while the I. P. T. was positive. 4) The results of thrombopenic phenomenon "in vitro" showed statistically significant relationship to those of the I. P. T. and the eye test. 5) No relationship was observed between the hemagglutination test and the other tests. Therefore, neither Middlebrook's method nor B. D. B. method seemed to be effective for the detection of skin sensitizing antibody to house dust in vitro.

The influence of upper respiratory infections on bronchial asthma has long been the object of discussion, but due to its complexity little is known about the mechanism of the effect. A study on the relationship of 6 kinds of upper respiratory bacteria, i. e, alpha-, beta- and gamma-hemolytic streptococci, Staphylococcus aureus, neisseria and hemophilus, to bronchial asthma was conducted and the summary of these results if given in this paper. 1) In evaluation of the intracutaneous skin test with the heat-killed vaccine of these 6 bacterial strains, a positive reaction is an induration of over 9 mm by the immediate test. 2) A higher positive rate was seen in asthmatics than in chronic bronchitis patients or in healthy subjects. 3) About 40% of the asthmatics showed a positive reaction to 2 or more of the bacterial vaccines. In a comparison of bacterial vaccine and house-dust extract skin tests, there were many positive reactors in the atopic type of bronchial asthma to house -dust extracts and in the infectious type to bacterial vaccines. The mixed cases showed positive reactors to both allergens. 4) In the intracutaneous skin reaction, no clear relationship was seen to age, years after onset, type of asthma (seasonal or perennial) and seasonal occurrence. 5) The relationship of the intracutaneous skin test to the bacteria isolaged from throat swabs or sputum obtained simultaneously varied according to the bacterial species, thus infering the difficulty in examination of past upper respiratory infections and isolation of bacteria.

The author has made a long term observation on the serum protein and the antibody in rabbits sensitized with Streptococcus Pyogenes. And the following results were obtained. 1). The change of Beta and Gamma globulin in serum was observed three times as the peaks during the sensitization. In the first two peaks they were in parallel with the titers of the specific precipitating antibodies (Boyden titer). In the last peak, the titer against the M-protein fraction was low, while the one against the C-polysaccharide fraction was not. 2). The Coombs antibody titers against the M-protein and C-polysaccharide fractions were increasingly observed or remained the same titer after the second change of peaks in Beta and Gamma globulin. 3). The antibodies or antibody-like substances reacting with homologous normal tissue extract were demonstrated. The author, however, could not necessarilly to confirm the increase of the titer during the experimental period. 4). The Coombs titer in the above reaction was rather low in the case of injuried tissue extract as the antigen than the normal tissue extract, while the Boyden titer remaind to be esentially identical. 5). In the long term observation on prolonged sensitization with Streptococcus, the corneal test were carried out and the positive reactions were observed with M-protein and C-polysaccharide fractions. Especially, the latter fraction revealed strong positive reaction. This fact suggests that the fraction plays a significant role in the immune response by Streptococcal infection or allergy.

In 487 cases of children's asthma treated at nine hospitals those belonged to Research Group of Pediatric Clinical Allergy, the following results of clinical studies were obtained. In the intermittent period of asthma no remarkable differences were found in peripheral RBC, WBC and Hemoglobin Index comparing with those of healthy children. Only slightly increasing tendency was observed in Hemoglobin Index and was normochromic. Increasing tendency of WBC in asthmatic attack is relative and the same was observed in RBC and Hemoglobin Index. Those were considered probably due to hemoconcentration. In attack, recovering and intermittent periods of asthma, no definite differences were observed in hemogram of WBC. Remarkable eosinophilia and slight lymphocytosis were noted. Even in intermittent period, definitely increased circulatory eosinophilia was observed and this was more dominant in attack period. In X-Ray studies of nasal sinus of asthmatic children, no meaningful incidencies and severity of abnormal findings were observed. In 60 to 70% out of all sputa and nasal smears of the cases in intermittent period and nasal smears in attack period, eosinophil cells were found, whereas in almost 100% out of sputa of the cases in attack period eosinophil cells were found.

Chest X-Ray films of 398 asthmatic children treated by Research Group of Pediatric Clinical Allergy were studied and were compared with those of normal chest X-Ray films of the same age group. Five members of this group checked the films and discussed. The following seven features were pointed out as peculiar findings of bronchial asthma. 1. Enlarged Thorax 2. Depression of Diaphragm 3. Decreased Cardiac Shadow 4. Narrow Cardiac Shadow 5. Enlarged and Heavy Hilar Shadow 6. Increased Radiolucency in the Bases 7. Increased and Heavy Vascularity in Hilar Region On these seven items, the frequencies were studied depending on the age and severity respectively. Some influencing factors on the abnormal findings and some causing mechanism were also discussed.
