    It has recently been suggested that epithelial damage participates in the development of bronchial hyperresponsiveness. In this study we investigated the relationship between the presence of clusters of desquamated respiratory epithelial cells(Creola body.CrB)in sputum and airway responsiveness. Sputa were collected from asthmatic patients and acetylcholine inhalation tests were performed to assess airway responsiveness. Smears of 100μl of sputum were spread in pairs of two glass slides. One slide was stained with Papanicolaou's stain, the other with Giemsa stain. CrBs were detected on the whole glass slide stained with Papanicolaou's stain and the CrB score was determined by summing up the points given to each CrB on a glass slide according to the number of epithelial cells composing one CrB. The CrB score was significantly higher in patients with CrBs than in patients without CrBs. The patients with higher CrB score(CrB score≥6)had significantly greater number of eosinophils in sputa than the patients without CrBs. These results suggest the participation of damage to the respiratory epithelium in the development of airway hyperresponsiveness in bronchial asthma. The detection of CrBs is easy and we consider it clinically useful in the estimation of airway hyperresponsiveness.

    To determine why midaglizole is effective in some patients with severe asthma, we investigated the inhibitory effects of midaglizole, prazosin or yohimbine on the BHT 920-, phenylephrine-, or noradrenaline-induced contractions of canine tracheal smooth muscle. After pretreatment with atropine(10^<-6>M)and propranolol(10^<-6>M)and precontraction with serotonin(3×10^<-7>M), the tracheal muscle showed contractile responses to the exogenous administration of both α_1 and α_2 adrenoceptor agonists. Every α antagonist inhibited these agonist-induced contractions. Inhibitory activity of midaglizole(10^<-4>M)for the α agonists was BHT-920>noradrenaline≧phenylephrine, while that of prazosin(3×10^<-6>M)was phenylephrine>noradrenaline>BHT-920. Moreover, yohimbine completely inhibited the contractions at the lower concentration of 3×10^<-7>M than that of other two antagonists. Our findings demonstrate that midaglizole dose-dependently inhibits airway contractions induced by α adrenoceptor agonists.

    We investigated the allergic factors of 94 allergic children;69 atopic dermatitis, 21 wheegy children, 3 urticaria and 1 milk allergy child under one year by multiple factor analysis. The results were as follows;1)Investigating by relative range ratio the 3 factors, RAST score to Dermatophagoides farinae and egg white, and family history of allergic disease, had strong influence to the existence of atopic dermatitis. 2)By multiple factor analysis type III, the 6 groups had tight relationship to atopic dermatitis group. (1)IgE RIST more than 61 IU/ml group. (2)Eosinophil counts more than 401/mm^3 group. (3)RAST score to Dermatophagoides farinae more than 0.70 PRU/ml group. (4)RAST score to egg white more than 0.70 PRU/ml group.

    The results were as follows;9 cases(36%)showed improvement, 10 cases(40%)mild improvement and 6 cases(24%)no improvement. Rice-RAST titers(PRU/ml)decreased in accordance with the clinical effects, namely most remarkably in the 1st group. Wheat-RAST titers also decreased noticeably in the 1st group even though they were eating wheat everyday, but increased in the 3rd group. In these cases the rice antigen may act on the production of IgE antibodies specific not only to rice but also to wheat as an initiator antigen. The serum IgE values increased in the 3rd group, but not in the other groups. In all groups, eosinophilia was seen, but no significant changes in condition after rice elimination diet were observed. Based on these results, the probable role of rice allergy in the pathogenesis of severe AD in Japan was discussed.

    Skin test, radioallergosorbent test(RAST)and nasal provocation test were carried out on 392 patients to detect house dust(HD)and mites as causative allergens. This study was undertaken to evaluate the difference between the results of skin test, RAST and nasal provocation test. The results were as follows:1. The frequency of positive intradermal test to mites was high regardless of age. But possibility of false positive results existed and this possibility increased with the patient's age. 2. RAST to mites was significantly higher than that to HD. In diagnosing HD allergy, measuring RAST to mites was more useful than measuring that to HD. 3. In pediatric patients, it could be assumed that measuring RAST to mites was the most applicable technique for diagnosing HD and mites as causative allergens. 4. The combination of intradermal test to HD and scratch test to mites was considered to be a useful screening skin test in diagnosing HD allergy. 5. Both the end point and reaction criteria of intradermal test were significantly correlated with the results of RAST and nasal provocation test.

    Acute hepatic necrosis was induced by a single i. v. injection of lipopolysaccharide(LPS)into C57BL/6 mice following immunization with syngeneic liver protein and adjuvant. Sixty percent of the mice died of massive hepatocyte necrosis within 48 hours of LPS injection. The serum lactate dehydrogenase and asparatate aminotransferase levels were markedly elevated. The fatal and hepatotoxic action of LPS was prevented by pretreatment with FUT-175(1.6mg/kg), a synthetic protease-inhibitor. The inhibitory effects of FUT-175 was not demonstrated when the agent was given to the mice 2 h after the administration of LPS. These results seem to indicate that the hepatotoxic effects of LPS are mediated by endogenous host mechanisms in which proteases play an important role.

    Broncasma Berna^[○!R](B. B. )prepared from killed polyvalent vacterial vaccine is used as a pleural irritant for pleurodesis on patients with spontaneous pneumothorax. The effects of B. B. on the complement system in vivo after injections into pleural spaces were investigated. Five patients with spontaneous pneumothorax underwent intrapleural injections of B. B. Increased body temperatures with positive C-reactive proteins and increased polymorphonuclear leukocyte counts in peripheral blood were observed in all patients. Serum levels of complement components(Clq, Cls, C4, C3 and B), hemolytic activity of complement(CH50)and plasma levels of C3a were gradually elevated after intrapleural injections of B. B. Analysis of pleural fluids from 4 patients showed that levels of C3a and C5a were increased compared with plasma levels, and an accumulation of polymorphonuclear leukocytes were observed. These findings suggest that intrapleural injections of B. B. activate the complement system and cause inflammatory reactions accompanied by an increase of C3a and C5a and an accumulation of polymorphonuclear leukocytes. Such inflammatory reactions might inversely induce the synthesis of complement components and promote the migration of polymorphonuclear leukocytes from bone marrow. The deposit of fibrin during these inflammatory processes might contribute to the development of pleurodesis. These inflammatory reactions might be mediated through the activation of the classical complement pathway by B. B.
