Background: In pediatric intractable asthma, there is occasionally an association with GERD (gastroesophageal reflux disease). It is not clear in which cases GERD should be suspected or how effective the GERD therapy is in treating the asthma. Methods: Twenty-seven preschool children (<6 years of age) suffering from recurrent asthma attack in spite of asthma therapy underwent 24-hour esophageal pH monitoring. We examined retrospectively the incidence of GERD and the effectiveness of famotidine in GERD positive patients. Results: 18 of the 27 patients (66.7%) had positive results (GERD positive group). In 12 of the 15 patients (80%) who underwent GERD therapy (famotidine), respiratory symptoms were decreased. In the GERD positive group, the incidence of acid reflux during waking hours was more frequent than during sleeping hours. In 8 of 12 patients (66.7%) in whom famotidine was effective, cough and wheeze often occurred during the daytime and corresponded with the time when acid reflux must commonly occurred. Conclusion: We conclude that children suffering from recurrent asthma attack in spite of asthma therapy must be examined for the presence of GERD.

Objective: To examine the causes of sequelae or death of patients who presented them subsequently to seizures that developed during theophylline administration. Methods: Among 334 patients who had been examined in Study (I), we wxtracted 56 who had sequelae or died subsequently to the onset of seizures and examined their causes. Results: Many patients had fever at the onset of seizures, many patients had neurologic predispositions prior to seizure onset, and only one patient was found to be free from fever and neurologic findings. At the onset of seizures, most of these patients had blood theophylline concentrations that were within or inferior to the target range (5-15μg/mL). Conclusions: We presume it necessary to pay heed to fever and neurologic predispositions in order to prevent sequelae or death that may occur subsequently to seizures that developed during theophylline administration.

Background: Atopy is usually defined as the genetic propensity to produce IgE antibodies (Abs) specific to common environmental allergens. The aim of this study was to evaluate impacts of the number of allergens examined on prevalence of atopy. Methods: Subjects comprised 116 healthy controls, 104 patients with asthma, 294 patients with COPD, 64 patients with sarcoidosis and 218 residents of Kamishihoro town. Total serum immunoglobulin E and antigen-specific IgE antibody levels for 26 common allergens were examined. Atopy was defined as positive IgE Abs specific to ≥ 1 allergen. We serially increased the number of allergens to define atopy from the most common allergens in order of frequency, and changes in prevalence of atopy were evaluated. In residents of Kamishihoro, betula pollen antigen was also examined, as this antigen is very common in the town. Results: The increasing prevalence of atopy was dramatically reduced relative to the number of allergens examined. Among residents of Kamishihoro town, 5 cases displayed specific IgE Abs to this allergen alone and prevalence of atopy was increased 2.3 percents. Conclusion: When atopy was defined as the production of specific IgE Abs to common allergens, roughly 10 of the most common allergens may cover common environmental allergens.

We reported a 4-month-old girl with biotin deficiency caused by amino acid formula. Two weeks after birth, she was diagnosed as having a milk protein allergy. After switching to amino acid formula from usual formula, her symptoms and laboratory findings became normal. About three weeks after the beginning of amino acid formula, she developed intractable skin erosions around the eyes, mouth, neck, and anogenital area. By measuring concentrations of some trace elements, she was diagnosed as having a biotin deficit, because of the organic aciduria and undetectable serum biotin concentration. Her serum biotinidase level was normal. Upon administration of oral biotin supplementation, all her symptoms and laboratory findings were dramatically improved. Since amino acid formula contains very few biotin, we should pay attention to biotin deficiency when infants receiving amino acid formula.

Background: Japanese cedar and cypress pollen share a common antigen. The cedar pollen season is followed by the cypress pollen season. However, both the clinical significance and involvement of cypress pollinosis in the treatment of the cedar pollinosis have not yet been clarified. Methods: The clinical efficacy of sublingual immunotherapy with cedar pollen extract for cedar pollinosis was evaluated during the cypress pollen dispersal season in Japan. In addition, the change in cypress pollen specific IgE antibodies of the patients with cedar pollinosis was examined before and after the pollen season. Results: Sublingual immunotherapy with cedar pollen extract did not improve the clinical symptoms of the cedar pollinosis patients combined with cypress pollinosis in the cypress pollen season. The cypress pollen specific IgE antibodies were found to demonstrate significant seasonal changes. Conclusion: The presence of cypress pollinosis should therefore be taken into consideration when planning the optimal treatment for cedar pollinosis. Sublingual immunotherapy with cedar pollen extract may not be effective for cypress pollinosis.
