Purpose: To investigate the prevalence of adult asthma and allergic rhinitis, and to analyze associations between smoking habit, obesity and disease in Kamishihoro town, Hokkaido. Methods: The Japanese edition of the European Community Respiratory Health Survey (ECRHS) Questionnaire was completed by 3096 residents (men: 1520, women: 1576) who ranged in age from 18 to 81. Results: Among the respondents, 12.9% of the males and 9.8% of the females responded "Yes" to the questionnaire item, "Wheezing at any time in the last 12 months" (defined as having asthma) and 17.6% of the males and 23.0% of the females responded "Yes" to the question, "Do you have any nasal allergies including hay fever?" (defined as having allergic rhinitis). This prevalence tended to be higher among younger respondents. Smoking habit and obesity were significantly associated with wheezing over the last 12 months, but not with allergic rhinitis. Conclusion: Smoking habit and obesity are significantly associated with asthma in Kamishihoro town, located in a rural area of Hokkaido, Japan.

Objective: We investigated the usefulness of skin prick test (SPT) for the diagnosis of egg white (EW) allergy in infants with atopic dermatitis who showed negative to EW CAPRAST, and followed up the EW-CAPRAST in this study. Subjects and methods: Data of negative SPT using Bifurcated needle (BN) were analyzed from the data of 202 atopic dermatitis infants, who had received SPT from January in 2001 to April in 2005. From the negative SPT value (average and standard deviation) positive SPT value was obtained. Among 202 cases, 89 suspected-egg allergy infants with negative IgE CAPRAST against EW at the time of first visit were recruited to examine the usefulness of SPT. Positive conversion of EW-CAPRAST was checked in 78 cases (65: egg allergy +, 13: egg allergy -) who had been followed up in our outpatient clinic. Results: Range of negative SPT control value (mean + 2SD) using BF among infants could be set as less than 2mm for wheal and/or 5mm for erythema. Among 89 suspected-egg allergy infants with negative EW-CAPRAST, 72 infants (80.9%) were diagnosed as egg allergy by the combination of elimination and provocation test, interestingly 39 infants (54.2%) showed positive SPT results. In the follow up study of 78 negative EW-CAPRAST cases, 47 EW-CAPRAST out of 65 egg-allergy cases turned positive later infantile period (mean EW-CAPRAST: 9.6 + / - 16.7Ua/ml at 9.9 + / - 5.6 months old). EW-CAPRAST of 7 cases in 13 non-egg allergies also turned positive in the follow up, however EW-CAPRAST titer was relatively lower compared to that of egg allergies (1.1 + / - 1.5Ua/ ml at 13.3 + / - 2.6 months old). Conclusions: We experienced fairly number of atopic infants with negative EW-CAPRAST at the first outpatient visit, who were later diagnosed as egg allergy. In about half of these cases, SPT egg-allergy infants, three quarter of EW-CAPRAST turned positive around 10 months old. EW-CAPRAST of atopic infants without egg allergy also turned transiently and slightly positive. In the conclusions, SPT seemed to be more useful than EW-CAPRAST for the diagnosis of egg allergy in early infantile period, however provocation test should be required for the definitive diagnosis in suspected-egg allergy infants without any proof of egg-sensitization.

Objective: In recent years, there have been sporadic reports of severe atopic dermatitis (AD) with hypoproteinemia and growth impairment. The present study was conduced in order to ascertain the characteristics of patients with severe infant AD with hypoproteinemia at the initial visit and their treatment courses. Subjects: Of AD patients younger than 1 year of age who visited the department over a 27-month period from March 2002, subjects were those with severe AD accompanied by hypoproteinemia. The clinical characteristics of these patients at the initial visit and the changes in symptoms and laboratory findings were statistically analyzed. Results: Of the total of 119 AD patients younger than 1 year of age visited to the department during the above-mentioned period, 15 patients had severe AD with hypoproteinemia. The height and body weight of approximately half of the patients were less than 3rd percentile, and 10% and more of the patients had severe hyponatremia or hyperpotassemia. The platelet count for 60% of the patients exceeded 800×10^3/μl. After visiting the department therapy involving the use of skin care products and topical steroids and the removal of exacerbation factors quickly improved dermal symptoms and laboratory findings. Conclusion: Severe AD is a disease that should be cautiously treated because of the risk of hypoproteinemia, growth impairment, electrolyte abnormalities, and thrombocytosis; however, it should be noted that appropriate treatments can improve this condition.

Background: To grasp the asthma patients' control level, an asthma control test (ACT^<TM>), an easy questionnaire, is getting to be used as the alternative method of asthma diary recording. ACT^<TM> is based on the patient's 4-week memory and the accuracy is not confirmed thoroughly Methods: Two hundred and eighty three visits of 88 asthmatics (male 29, female 59) were subjected. The accuracy of ACT^<TM> to detect "truly good control" was analyzed by comparing with the asthma diary (symptoms and PEF were recorded). Results: Sensitivities, specificities (chi square values) for extraction of truly good control were 0.88, 0.42 (23.7) by the threshold of ACT^<TM> ≥ 23, 0.77, 0.51 (18.1) by ACT^<TM> ≥ 24, and 0.54, 0.69 (12.9) by ACT^<TM>=25, significantly (p<0.001, respectively). When PEF% predicted ≥ 80% was added as the second factor, the chi square values improved to 29.9 by ACT^<TM> ≥ 23 and to 30.3 by ACT^<TM> ≥ 24 significantly (p<.0.001, respectively). Conclusion: Addition of PEF measurement on visit on ACT^<TM> may help understanding the asthmatic patient condition.

We describe a19 year-old woman who was diagnosed as systemic lupus erythematosus (SLE) after abortion. She had taken anti-convulsants for epilepsy since she was 8 years old. Induced abortion surgery was performed at six weeks in her pregnancy. She showed pyrexia and a general rash 2 days after the abortion. She was introduced to our hospital because the administration of antibiotics was not effective. Since the anti-convulsants had been changed after pregnancy, we returned to those administered before pregnancy and followed her up. Her eruption improved, but she became aware of thirstiness and dry eye. She was diagnosed as Sjogren syndrome by ophthalmologic examination, lip biopsy, and elevation of an anti-SS-A antibody and an anti-SS-B antibody in the serum. Since we could not rule out SLE because of the low concentration of complement activity in blood, we followed her up carefully by checking serum markers of SLE. Protein urine developed after the improvement of the eruption 2 weeks later. Low complement activity was recognized and double stranded (ds)-DNA antibody became positive. In addition to these findings, she had an episode of hypersensitivity to sunlight and was therefore diagnosed as SLE. Since induced abortion and drug eruption might be associated with the onset of SLE, the case is thought to be a valuable from the view point of understanding the mechanism of SLE onset.
