Background: Total serum immunoglobulin (Ig)E levels and peripheral blood eosinophil counts are widely examined to evaluate patients with various allergic diseases. Asthma and allergic rhinitis often coexist. However, the significance of these indices for asthma and rhinitis under consideration of the status of co-existence has not been fully elucidated and was therefore examined in the present study. Methods: Subjects comprised 347 adult residents in Kamishihoro town, Hokkaido. Relationships between two indices and asthma, rhinitis and their coexistence were analyzed. Results: Serum IgE (sIgE) levels were significantly higher in asthma with (p<0.01) or without (p<0.01) rhinitis, regardless of atopic status, but not in rhinitis alone. Peripheral eosinophil counts were significantly higher only in asthma with rhinitis (p<0.005). Conclusion: Compared with rhinitis, non-antigen-specific IgE production may contribute more to elevated levels of sIgE in asthma. In addition, the significance of sIgE and peripheral eosinophil count as indices of evaluating asthma and rhinitis might differ.

Case 1: 67-years-old woman with pollinosis noticed oppressive feeling of chest and back, and heart burn after accidental ingestion of her dental filling and dental treatment. Oral famotidine did not improve her symptom. Her peripheral blood eosinophils increased to 38.0%. As for the specific IgE, only cedar and cypress were positive. Case 2: a 42-years-old-woman with pollinosis and asthma repeated urticaria, heart burn, diarrhea and peripheral eosinophilia (25%). At her first visit to our department, her blood eosinophil increased to 52.9%, her serum IgG markedly increased in polyclonal pattern and overt all subclasses. The specific IgE was positive only for cypress. Because they revealed a remarkable infiltrates of eosinophils in the mucosa of alimentary tract, we diagnosed the 2 patient as eosinophilic gastroenteritis. In case 1, based on the history and patch-test-positive finding of formalin and 2-hydroxyethyl methacrylate, we diagnosed the two may be causative allergens. In case 2, based on the patch-test-positive finding of garlic and sesame and improvement after removal of the two allergens, we diagnosed the two may be causative allergens. Although causative allergens of eosinophilic gastroenteritis are almost unknown, some cases are reported to be determined the allergens of foods and drugs. In our cases, patch test was useful to identify the allergens.

A 47 years-old woman suffering from baker's asthma for several years developed anaphylaxis when she was walking hard after taking wheat. On the provocation test, neither wheat alone nor exercise alone induced any symptoms. The combination of exercise, wheat and aspirin induced urticaria and marked elevation of blood gliadin levels. According to the high titer of ω-5 gliadin specific IgE in her serum and the result of challenge test, we diagnosed wheat-dependent exercise-induced anaphylaxis (WDEIA). By means of western blotting of soluble and insoluble wheat proteins, we detected several bands which were supposed to be β-, γ- and ω-5 gliadin by their relative molecular mass. Wheat gliadins might be causative allergen of both baker's asthma and WDEIA in our case.

The patient was an 83 Year-old woman with purpura on her right foot and on both crura starting from the end of November 2005. First, she took medical advice from a nearby doctor, but it did not improve. Since blisters were formed besides purpura, she visited Nagoya City Midori Municipal Hospital. When she visited, she had many purpura on her both legs, her right lower crus and her right foot was swelling with redness, blisters and erosion. Histopathological finding showed leukocytoclastic vasculitis, deposit of fibrin and hemorrhage. Only Empedobacter brevis was cultured from right feet lesion. This strain was sensitive to minocycline hydrochloride. Her purpura and right foot swelling were improved just after internal use of minocycline hydrochloride, so Empedobacter brevis infection was speculated to be a trigger, or at least concerned to this Anaphylactoid purpura.
