    The agar dilution technique which has been used at our laboratory for several years was converted. Briefly, serial two-fold dilutions of the antibiotic are prepared and incorporated into GAM agar (Nissui). The inoculum is prepared as follows : Almost all strains of anaerobes except slower growing strains is grown in GAM broth (Nissui) for about 6 hours anaerobically. A 6-hour culture in GAM broth has the turbidity of a half of MC FARLAND no. 1 (105 to 106 viables cells/m1). When a 24-hour culture is employed, it is diluted in UENO'S diluent (without agar) or 0. 05% yeast extract solution to the same density above.
    The inoculum is applied by means of a multipurpose apparatus for rapid inoculation.
    The susceptibility plates are incubated at 37°C in anaerobe jars (Steel-wool copper sulfate method; CO2 10%, N2 90%) for 24 hours.
    The MIC of each strain is read as the highest dilution of drug yielding no growth.
    This procedure is better at the point of the rapidity, simplicity and reproducibility.

    A study was made on the anti-inflammatory action of erythromycin (EM).
    1) EM exerted a slight inhibitory effect on increased vascular permeability induced by histamine in rats. Cephalexin (CEX) was used as the reference drug, and even when used of high doses, it exerted no inhibitory effect. EM was, therefore, considered to have an action to inhibit increased vascular permeability irrespective of its antimicrobial action.
    2) EM exerted a moderate inhibitory effect on the paw edema by carrageenin and dextran, while CEX exerted no inhibitory effect. This antiedematous action was dose-dependant. On the other hand, this action was not manifested in the adrenalectomized rats. The anti-edematous action of EM was, therefore, considered an indirect action resulting from the increased adrenocortical activity.
    3) EM was not inhibitory to ultraviolet erythema in guinea pigs.
    4) EM was slightly inhibitory to the granuloma proliferation in rats when tested by the cotton pellet method.
    5) EM exerted only a slight inhibitory effect on the primary swelling of treated foot in the adjuvant arthritis after tested for 3 weeks, whereas a potent inhibitory effect on the secondary inflammation. 6) EM exerted only a slight inhibitory effect on the heat denaturation of bovine serum albumin.
    The antibiotic, however, proved to exert a fairly potent inhibitory effect on the heat-induced lysis in rat erythrocytes.
    7) By RANDALL and SELITTO's method in rats, EM did not raise the pain threshold in normal foot at all, but slightly raised that in inflammatory foot.
    These experimental finding disclosed that antibiotics of the macrolide group-especially erythromycin-exert a slight anti-inflammatory action and the mode of action of EM is similar rather to that of steroidal anti-inflammatory agents than non-steroidal one. It may be considered that the anti-inflammatory action of EM is exerted through the indirect effect due to adrenal stimulation, apart from their antimicrobial activity.

    It is important that the host-parasite-drug relationship is taken into consideration in the case of chemotherapy. The immunosuppressive effect of CP and RFP was reported recently. Therefore, the influence of antibiotics on infection, tuberculin reaction to BCG immunization and the lysosomal enzyme activity of macrophages and WBCs was investigated. The results are as follows :
    1. The pre-treatment with CP, KM, TC and PCG in mice is brought about easy infection to St. aureus.
    2. The treatment with RFP, CP and TC in guinea pigs suppresses the tuberculin reaction to BCG immunization.
    3. RFP and CP in mice and CP, RFP, TC and SM in guinea pigs obviously suppress the enzyme activity of macrophages.
    4. The treatment with CP, RFP, TC and SM in guinea pigs suppresses the peroxidase activity of WBCs.
    5. The above results should be taken into consideration in the case of the treatment of large dosis and the long term treatment and opportunistic infections.

    In 1962, E. L. COMPERE reported on the successful use of topical treatment in a few cases of osteomyelitis and postoperative pyogenic bone and joint infections. We have continued to use this method, and most of the 70 cases of osteomyelitis so treated have healed. We began to use Alevaire and Urokinase in the antibiotic solutions instilled for the treatment of osteomyelitis or postoperative infections by the closed irrigation technique. Seventy patients were treated by this technique, and 65 patients did not show recurrences one year or more after treatment. Thirty-one of 70 infections were caused by hematogenous infection, and 18 of 70 infections were postoperative. Twenty-eight infections were caused by Staphylococcus aureus, 12 infections were caused by Pseudomonas aeruginosa and 6 infections were caused by Escherichia coli. Duration of infection prior to closed irrigation was from 5 days to 50 years, and 24 cases were 10 years or more. Duration of irrigation was from 5 days to 40 days. Most of them were from 11 days to 30 days. We made a special circulation system using double tube to keep the irrigation from obstruction of the tube. We call it 4 channel method. Since then we have not worried obstruction of the tube or leak of the solution.
    We examined blood concentration of ABPC, CBPC, SBPC, CET and TPG during closed irrigation. But the blood concentration of these antibiotics were very low, from zero to 31. 9 μg/ml. On the contrary, however, we found high concentration of antibiotics in the egress fluids, from 75. 7 μg/ml to 1, 577. 2μg/ml.
