  • -THREE-DIMENSIONAL MEASUREMENT OF TMJ BONY SPACE-

    The purpose of this study is to establish the criteria for the diagnosis of the positional relationships between condyle and articular fossa of the patients with so-called TMJ Arthrosls.
    Three-dimensional measurement and reconstruction of the TMJ bony space were made using a simultaneous lateral tomography and computer.
    Eighty patients with TMJ Arthrosis and eleven persons of individual normal occlusion without subjective and objective symptom were put into this study. Tomographic examination was performed with the teeth in the intercuspal position.
    Condyle-Fossa Distance (CFD) was determined by the shortest distance between one point matrix of condyle and another point matrix of fossa.
    Mean value and standard deviation of three-dimensional measurement was 2.02mm±0.23mm to the actual distance of 2.11mm. The change of distribution of CFD frequency by projective angle was negligible.
    The following analyses and investigation were performed with special reference to the CFD of both sides.
    1) The patterns of bilateral distributions of CFD frequency were classified in 6 types.
    2) The correlation between bilateral distributions of CFD frequency and abnormal radiographic finding of bone components was investigated.
    3) The correlation between bilateral distributions of CFD frequency and clinical symptoms (Grade-1. 2. 3.) was investigated.
    From these studies, following results were obtained:
    1. No CFD less than 1.0mm was found in the control group and bilateral distributions of CFD frequency were corresponding in 9 of 11 cases.
    2. CFD less than 1.0mm and/or distributions of CFD frequency not corresponding bilaterally were found in 63 of 80 cases (78.8%) in the patients group.
    3. The cases in which distributions of CFD frequency were not corresponding bilaterally revealed radiographic bone changes at a high rate (68.0%).
    4. In the groups those CFD of unaffected side was smaller than that of affected side (type 3&6), limitation of opening mouth with pain (Grade-3) was the most predominant clinical symptom (18/35, 51.4%).
    From these results, it was suggested that distributions of bilateral CFD frequency were correlated with radiographic bone changes and clinical symptoms.

    Wistar strain male rats weighing about 100g were used in this study. The heads of the animals were irradiated with single dose from 25rad (0.25Gy) to 1000rad (10Gy) under the following conditions; 100kVp, 4mA, Filter: 0.1mmCu+0.5mmAl, FSD: 30cm, HVL: 3.5mmAl. The dentin and odontoblasts on the labial sides of maxillary incisors were observed with light-, fluorescence-, electron-microscope and microradiography.
    The following results were obtained:
    1) The minimal absorbed dose affecting the dentin formation was 100rad (1Gy), which was the lowest dose compared with the previous investigations.
    2) Dentin hypoplasia was found remarkably as the absorbed dose increased.
    3) The most radiosensitive cells were prefunctional preodontoblasts which were differentiating papilla cells with polarizing nuclei and without capacity of predentin-formation, and the second radiosensitive were preodontoblast precursors.
    4) The alterations of nuclei and cytoplasm became severe as the absorbed dose and the lapse of days after irradiation increased.

    Thirty five cases of the post operative maxillary cyst were examined and discussed radiologically with reference to the operative and histopathological findings. Prior to the examination, the radiographic appearances of the contralateral maxillary antral regions operated in the past, without no symptom, were analyzed on the frontal and sagittal tomographs, and conventional radiographs including orthopantomographs. The maximum width of the inferior nasal meatus and the vertical dimension of the orbit were measured and compared with that of 18 patients without history of the radical operation of the maxillary antrum.
    And then, the form, the location and the outline of both the postoperative maxillary antrum and the cystic lesion proved by surgical investigation, were assessed and compared with each other.
    The following results were obtained.
    1. The inferior nasal meatus of the patient with history of the radical operation of maxillary antrum was significantly wider than that of the patient without the operation (p<0.0001).
    2. Postero-anterior aspects of the postoperative maxillary antrums were classified into three types such as reduction type, depression type and maxillary antral type.
    3. In the case of a good recovery from the past surgical operation, post operative maxillary antrum was placed in contact with inferior and/or middle nasal meatus.
    4. Postoperative maxillary antrum was in communication with the adjacent nasal cavity through the one or more opening in the mucous membrane, while the cystic lesion was enclosed with the soft tissue, probably a cystic lesion was enclosed with the soft tissue, probably a cyst wall similar to the unsound mucous membrane.
    5. The cysts rarely showed egg-shell like bony expansion, even if the lesions extended into the surrounding soft tissue. They showed, sometimes the bony defect at the anterior or posterior surface of the maxillary bone.
    It was suggested that the growth potential of the post operative maxillary cyst was lower than the other cysts of the jaws and the cyst scarcely extended beyond the spaces, originally ocupied by the maxillary sinus. Differential diagnosis between the post operative maxillary cyst and the post operative maxillary antrum could not be made radiographically untill the communication between the nasal acvity and the questionable radiolucent area was proved by the frontal tomography.

    At the time of radiation therapy for head and neck tumors, not only the major salivary glands but also many salivary glands are included in irradiation field. Consequently xerostomia are manifested in many of patients by degrees and oligosialia or asialia is objectively recognized. After the interstitial radiation therapy with or without external radiation therapy for the treatment of cancer of tongue and floor of the mouth, patients complain pain and/or swelling in submandibular region as often seen in the sialolithiasis.
    Sialographic changes of the major salivery glands after radiation therapy were analysed.
    Twenty six patients undergone radiation therapy for head and neck cancers were put into this study. Seven had carcinoma of oropharynx, six had carcinoma of tongue, three had carcinoma of nasopharynx, three had carcioma of salivery glands and seven has other malignant tumors.
    The contrast medium used is 76% Urographin.
    Sialographic findings were investigated concerning two major salivary glands: submandibular and parotid gland.
    Submandibular glands: Stricture of Warton's duct was the main early findings after interstitial radiation therapy. Rupture of Warton's duct and/or atrophic change of submandibular gland often observed with time.
    Parotid glands: No remarkable sialoglaphic change was detected in any time after external radiation therapy of 4000 rad (40Gy). Stricture of Stensen's duct was the most predominant findings at 6 months later since the completion of external radiation therapy of 6000 rad (60Gy), and the findings further progressed with time. After the completion of external radiation therapy of 8000 rad (80Gy), rupture of Stensen's duct and atrophic change of parotid gland were observed.
    As survival rate of head and neck cancers was increased by radiation therapy, various side effects were also recongnized. Careful consideration must be needed on the treatment planning so as to minimize the late damages.

    Adenoid cystic carcinoma is a malignant salivaly gland tumor that occurs most commonly in the hard palate.
    A case of adenoid cystic carcinoma in an 85-year-old male is presented with special reference to radiographic findings. The patient was reffered to the Gifu College of Dentistry Hospital for prothodontic treatment of ill-fitting lower denture.
    Radiographes revealed that the mandibular bone was almost entirely destroyed, presenting “moth-eaten-appearance”.
    A central tumor was suspected from the clinical, radiographic and histopathological examinations.

    It is described by Worth in 1969 that von Recklinghausen's disease reveals bone changes of the jawbones, which is considered rather rare. Studies were made in 3 cases of von Recklinghausen's disease (neurofibromatosis) of the jaws and 37 cases of the jaws were reviewed in the literature from 1937 to 1982 in Japan.
    Results are as follows:
    1) The authors' three cases and 17 cases in the literature showed changes of jawbones.
    2) Most of cases revealed unilateral changes of the bone.
    3) The mandibular angle showed deformity in the authors' three cases and 11 cases in the literature.
    4) Radioloucent changes of the mandibular notches were found in the authors' three cases and in a few cases in the literature.
    5) It was rare to show cyst-like radiolucencies of the radiogram.
    6) Enlarged mandibular canals around mental foramina were important radiographic findings, which tumor development was suspected.

    Surveying and recording the actual state of X-ray practice at periodic time intervals have a very important meaning from the standpoint of radiation protection. Designating the one week period of November 24 to 30, 1982 as a survey period, an actual state study on dental radiology practice was carried with the cooperation of all dental clinics and dental departments of general hospitals throughout Japan.
    Comparison with the results of the previous study (1976) showed an improvement in the following areas: construction of exclusive X-ray rooms (25.7%), construction of darkrooms (47.4%), dissemination of automatic processors (47.1%), use of protective aprons (63.1%) and carrying of personnel monitoring dosimeters (19.5%).
