Verrucous porokeratosis is a rare variant of porokeratosis characterized by the raised patch showing multiple cornoid lamellae with epidermal hyperplasia. In this paper, we report a case of verrucous porokeratosis arising in the buttock. The patient was a 37-year-old man with a 21×17 mm, reddish keratotic plaque on the right buttock near the gluteal cleft. Histopathological examination of the resected specimen showed acanthosis and verrucous hyperplasia with prominent broad, confluent and multiple cornoid lamellae. Epidermal hyperplasia with multiple cornoid lamellae was present throughout the entire lesion. We reviewed the current literature on verrucous porokeratosis and speculated that this rare variant might have a higher risk of malignant transformation than that of the other types of porokeratosis.
