    We have previously experienced patients with pemphigus vulgaris in remission, who had high anti-desmoglein (Dsg) 3 antibody index and negative indirect immunofluorescence (IIF) and reported the analyzed results of the autoantibodies with a focus on the discrepancy between the anti-Dsg antibody index and IIF. Recenty, we investigated 76 pemphigus patients treated at our hospital between 2008 and 2020, whose courses could be tracked to examine their prognosis and the relationship between anti-Dsg antibody index and IIF. Of the 21 cases with positive anti-Dsg antibody index and negative IIF, most were in remission with no treatment or with a minimal amount of oral steroid. These data suggest that following not only the anti-Dsg antibody index, but also the IIF result, is important in deciding the course of treatment for some patients with pemphigus.
