    A histopathological study was made in 7 autopsy cases on sequential changes of thrombi formed within the esophageal varices after the endoscopic injection sclerotherapy by use of 5 % ethanolamine oleate and the reappraisal was made on the application of this sclerotherapy. 1) In 3 autopsy cases died within 1 month after the injection sclerotherapy, the injected esophageal varices were completely obliterated by red thrombi, in which almost no organization from the variceal walls was identifiable. 2) In one autopsy case died 2 months after the injection sclerotherapy, some of the intravariceal thrombi started organizing partially, and in the other autopsy case died 4 months after the therapy, organization of these thrombi progressed considerablly, showing complete organization of the thrombi within smaller varices, though in the thrombi within larger varices the organization remained to be partial. 3) In each of autopsy cases died in 9 months and 1 year after the injection sclerotherapy, respectively, a red thrombus was no longer identifiable in the injected varices. The varices obliterated by the organization of the thrombi were completely collapsed. 4) Recanalization within these thrombi appeared to start 4 months after the injection sclerotherapy and no evidence of recurrent esophageal varices was shown a year after the therapy. From these findings, organization of thrombi formed within esophageal varices by the present sclerotherapy appeared to progress very slowly and require about 9 months to complete. It was not probable that esophageal varices would recur through recanalized vessels in organized thrombi with at lease one year after the therapy.

    46 Cases of gastric ulcer appearing to occur after drug administration (drug-induced gastric ulcer) were classified by endoscopic observation as follows : 1) Type exhibiting multiple small ulcers in the antrum. 2) Type exhibiting a single ulcer or a few ulcers in the angle and body. 3) The so called "trench ulcer" type. 4) Type resembling an ordinary chronic peptic ulcer. Drug-induced gastric ulcers demonstrate a different form and course depending upon whether the patients have a history of chronic gastric ulcer or not ; 1) In patients who had no history of chronic gastric ulcer, the ulcers appeared as 1) through 3) of our classification and healed within 2 months in most cases. 2) In patients who had a history of chronic gastric ulcer, the ulcers appeared as 4) of our classification. These ulcers had tendency to require long time until healing. Drugs inducing ulcers consist mainly of antipyretic-analgesis and some cnemotnerapeutics, anticoagurant and peripheral vasodilator on a few occasions.

    We compared 20 chronic hepatitis cases with suspected liver cirrhosis on the liver function tests with other chronic hepatitis cases, analyzing liver function tests, clinical and peritoneoscopic findings and liver biopsy specimens. On the clinical findings, vascular spider and palmer erythema were found more frequently in the 20 cases with suspected cirrhosis than the other chronic hepatitis cases . In the liver function tests, we investigated 5 items, GOT/GPT ratio, ChE, ZTT, y-globulin and ICGR15 values and obtained statistically significant differences between the 20 suspected cirrhosis cases and the other chronic hepatitis cases . These cases showed more frequently diabetic changes of glucose tolerance tests and splenomegaly of liver scintigram . Peritoneoscopically, many small depressions were found in 10 cases and lobular distortion was higher than in the other chronic hepatitis cases. Under microscope, the enlargement and inflammation of Glisson's capsula, piecemeal necrosis and bridging necrosis were found in many cases. We thought that the causes of the changes in liver function tests in 20 cases suspected liver cirrhosis were due to severe damage of liver cells, strong inflammation of portal area and reduced hepatic blood flow.

    Inprovement of the ultrasonic system which we previously reported, was made for the diagnosis of intra-abdominal lesions and its availability in clinical use was studied. With several problems of the former models in mind, the system has been improved as follows. The scope provides a wide view field of 45° forward obliquely from the axis in order to secure scanning under visual control. The axis of the probe can be changed from -90° to 90° horizontally by a bending controller to cover wider range of scanning in the abdominal cavity. The display unit can produce 180° as well as 90° dynamic sector images. In resolution study using autopsied materials, it was proved that echograms of livers bearing tumors obtained by this system coinsided well with the macropathological appearance of the cut surface. In clinical use, the scope could be easily handled and scanning could be carried out with no trouble under visual control. From the results comparing the echograms obtained by this system with those by a conventional one, this system can be assumed to be useful mostly when liver tumors are suspected but cannot be detected by any other imaging. It might be also helpful in determination of the boundary for resection of liver tissue bearing tumors on surgery. In diagnosis of diseases of the biliary tract, this system cannot be considered to surpass conventional ultrasonic ones, though echograms of gall stones and polypoid lesions of the gall bladder were excellent. Although the head and body of the pancreas were distinctly visualized by this system, further study must be done to decide its availability in diagnosis of pancreas diseases.

    Since carcinoma of the esophagus is usually disseminated by the time when the diagnosis is made, treatment is often palliative. It is similar in cases of carcinoma of the cardia. The Microwave Tissue Coagulator (M. T. C) had been developed by K. Tabuse and Co-workers. Expecting effects of tissue coagulation and destruction, we applied Endoscopic Microwave Coagulation Therapy (E. M. C. T) for 8 cases of esophageal malignant stricture. Four cases were carcinoma of the esophagus and the others were carcinoma of the cardia. All of them were judged inoperable because of distant metastasis or other reasons. As a result, significant symptomatic, endoscopic and radiographic improvements were noted in 7 cases (88%). Dysphagia was improved in 7 cases, and the endoscope passed through the stricture in 4 cases where it could not before E. M. C. T. We used M. T. C with higher power, mainly 70-100 W, than that in fundamental experiments already reported, and we could obtain more effective coagulation without any major trouble. A newly introduced dissociation apparatus was effective for preventing coagulated tissues from sticking to the monopolar electrode. One of the characteristics of M. T. C different from an electro- or laser coagulator is that the monopolar electrode is punctured into the tissue, and the tissue can be coagulated accurately and safely. We concluded that E. M. C. T for malignant stricture of the esophagus may provide a new promising palliative treatment.

    There is no detailed report on acute gastric lesions following the administration of Mitomycin C (MMC) into coeliac or hepatic artery. In order to elucidate the effects of 20 mg of MMC administration into coeliac or hepatic artery on the gastric mucosa, gastroscopic examinations were performed in three patients with primary and three patients with metastatic hepatic cancer, before and after each injection for 11 times in all. Furtheremore, the effects of concomitant administration of Angiotensin II (A-II) were studied. Gastroscopic findings one week after the administration of 20 mg MMC revealed multiple erosions (edematous polipoid lesions with central brown erosions) in eight cases, solitary erosion in one, and a gastric ulcer in one. Histologically, these gastroscopic alterations ranged from groups I to II when classified by the Japanese Research Society for Gastric Cancer. Gastroscopic findings after MMC with A-II infusion in three patients, before and after four trials, revealed multiple erosions in two patients and solitary erosion in one patients. No particular finding was noted in one patient. Histology of these findings was of group II. These results may suggest that the acute gastric lesions by the high dose of MMC might be due to its direct action on the gastric mucosa and/or by the circulatory dis-turbance originated from intimal lesions of arterioles of the stomach. Thus, the infusion of A-II might in part protect the gastric mucosa from the toxic effect of MMC.Gastroenterological Endoscopy

    Intravenous glucagon was studied as a premedicative drug for endoscopic retrograde cholangiopancreatography with butropium bromide as a control drug. As results the following were observed. 1) There were no differences in the peristalsis of the stomach and in the hypotonicity and peristalsis of the duodenum between glucagon and butropium bromide. 2) Concerning side effects, dry mouth, accommodation disturbance, dysuria, abdominal pain and palpitation were frequently observed in the butropium bromide group. 3) It was confirmed glucagon can be used as a premedicative drug for an endoscopic retrograde cholangiopancreatography as well as anticholinergic drugs, and also for cases in which the anticholinergic drugs are contraindicated. Glucagon will be widely applied in this area because it has few side effects.

    Peritoneoscopic examination was performed in 9 cases with gastric cancer and the usefulness of peritoneoscopy for determination of operative indication was evaluated. Peritoneoscopy was accurate for diagnosis of extension of gastric cancer, especially in observation of serosal invasion, peritoneal dissemination and metastasis to the liver. In addition, there was a case diagnosed more accurately by peritoneoscopy than by surgical operation. It is concluded that peritoneoscopy is useful to determine operative indication in such a case without obvious cancerous ascites, metastasis to other organs by imaging, and distant metastasis.

    Gastric syphilis is a rare disease of the stomach, and may be confused with neoplastic condition clinically. In most cases previously reported, an accurate diagnosis is made only after surgery. The case we reported seems worth recording. A 29-year-old professional sportsman with severe epigastric pain who had been recom-mended to have surgery for probable malignant lymphoma consulted our clinic. Endo-scopic examination showed a luminal narrowing with thickened mucosa of the gastric antrum. X-ray examination showed poor distensibility and various sized nodular mucosal changes in the antrum, but irregularity and rigidity of the antral wall were unremarkable. Repeated biopsy could not reveal malignant findings, and a positive result of TPHA screening test suggested the probability of gastric syphilis. Upon detailed questioning, it was noted that he had had symptoms of syphilistic infection. Immediately after making a suspicion of gastric syphilis, the patient was placed on penicillin. Epigastric pain was relieved in a few days and endoscopic findings were improved noticeality in two weeks.

    A case of stomach carcinoma, which was difficult to make an accurate diagnosis because of its unusual macroscopic findings and very low cellular atypism, has been reported. A 50-year-old man was admitted to Osaka Medical School, because of his epigastric pain. A Borrmann type III stomach carcinoma, lymphosarcoma or reactive lymphoid hyperplasia were suspected by several series of barium meal and endoscopic examinations of the stomach which showed a shallow ulcerous lesion accompanying a low enbankment, but 19 biopsy specimens taken by three biopsy procedures revealed gastritis only (Group I & II). A tumorous lesion showing a low platteau like elevation with a shallow depression on the top and accompanying conversing folds. The size was 4.5 cm by 4.0 cm, the tint and the texture of the surfance were almost the same as the normal mucosa without mucous coat. It was found on the anterior wall of the resected stomach. Histologically, a greater part of the lesion showed extremely low cellular atypism to diagnose carcinoma, but the appearance of actual invasion to the surrounding tissue at the margin of the lesion and lymphatic invasion led us to make the diagnosis of carcinoma.

    A 54-year-old female had been followed up for rheumatoid arthritis. She was admitted to Nagasaki Municipal Hospital with a chief complaint of epigastric pain and watery diarrhea. Gastroendoscopic examination showed multiple macular, spotted redness localized in the antral region, and biopsied specimens from the reddened mucosa revealed amyloid deposits within lamina propria of the mucosa. Endoscopic examination of the duodenum, sigmoid colon, and rectum also showed reddness, and biopsy revealed amyloid deposits. This case was diagnosed as secondary amyloidosis. Endoscopic findings of gastric amyloidosis reported in Japan was reviewed.

    Although submucosal cyst of the stomach is a rare condition, the high incidence of the association of this lesion with gastric cancer is widely recognized. We herein report 3 cases of submucosal cyst associated with multifocal gastric cancer (Table 1). Case 1, a 57-year-old male : multiple submucosal cysts, linear ulcer scar, and type IIb cancer (m) in the upper body of the stomach in addition to type IIc cancer (m) at the angulus (Figure 3). Case 2, a 75-year-old male : multiple submucosal cysts, type IIc cancer (m) and type IIc+III cancer (sm) in the lower body of the stomach (Figure 7). Case 3, a 77-year-old female : a submucosal cyst in the body, advanced cancer (pm) in the posterior antrum, type ha cancer (m) in the anterior antrum and atypical epithelium (group III) at the angulus of the stomach (Figure 11). With regard to the relation between gastric submucosal cyst and cancer, we discussed on the pathogenesis of submucosal cyst and frequency of this lesion complicated with gastric carcinoma.

    A 47-year-old female was admitted to Akita University Hospital because of melena. Uppergastrointestinal series and endoscopic examination revealed a large mass with a central depression in the duodenum. Biopsy specimens showed no malignancy. Ultrasonic examination showed an echogenic mass, and CT study showed a low density lesion. Abdominal angiography showed a hypovascular mass. Resected specimen showed a duodenal lipoma, 5.5×5.0×3.5 cm in size.

    A patient of chronic pancreatitis whose addominal pain was relieved by endoscopic elimination of protein plugs was presented. The patient was 57-year-old male without alcohol history. Cholecystectomy was done for cholecystolithiasis in April 1979 and endoscopic sphincterotomy was done in September 1979. However, epigastralgia was continued and he visited our clinic. 5-amylase, u-amylase was within normal range and PS test showed normal pancreatic exocrine function except for mild decline of bicarbonate concentration. ERP showed moderate abnormality compatible with chronic pancreatitis and radiolucent shadows in the pancreatic duct suggestive of protein plugs. Protein plugs were observed in pure pancreatic juice obtained by duodenoscopic cannulation. In the other hand, each parameter of pure pancreatic juice (volume, amylase, bicarbonate, protein and lactoferrin concentration) was within normal range. After pure pancreatic juice collection via duodenoscopic cannulation, epigastralgia from which he had been suffering for a long time was relieved. Therefore, this method seems to be useful not only as a diagnostic procedure but also as a therapeutic procedure.
