    We conducted this study to clarify the characteristics of cancer without lymphnode metastasis. Five hundred and three cases of early gastric cancer who underwent gastrectomywere histopathologically analized. The results were as follows: The indices of cancer without lymph nodes metastasis were 1)In type I, histologically differentiated type of mucosal cancer less than 40mm in size, or differentiated type of cancer less than 52mm in size invading the submucosa less than 4000μm in depth, 2)In type IIa or IIb, all of the differentiated mucosal cancer, 3) In type IIc without ulceration, all of the differentiated mucosal cancer, differentiatedtype of cancer invading su.bmucosa less than 1800μm in depth, or all of the undifferentiated type of mucosal cancer, 4) In type IIc associated with ulceration, differentiated type of mucosalcancer with the ulceration limited to the upper and middle third of submucosal layer, differentiated type of cancer with the ulceration limited to the upper and middle third of submucosal layer invading subrnucosa less than 1000μm in depth, or undifferentiated type of mucosal and submucosal cancer with the ulceration limited to the upper and middle third of submucosal layer. In conclusion, it was considered to be possible to expand the indications for endoscopicmucosal resection of early gastric cancer.

    A 64-year-old male underwent distal gastrectomy(D2)to treat type IIa+IIc of gastric cancer on the pyloric anterior wall. The resected specimen suggested sm carcinoma. On the immunohistological staining, the specimen was storongly positive for alpha-fetoprotein (AFP).The serum AFP level was increased to 170ng/ml, and was normalized after surgery. To date, no liver metastasis has been detected. In patients with AFP-producing gastric cancer, the incidence of liver metastasis is high. In many cases, this type of gastric cancer is detected as advanced cancer. In this study, we report a patient with early stage of AFP-producing gastric cancer.

    A 50-year-old man with a chief complaint of abdominal distension admitted to ourdepartment for further examination. Endoscopic study revealed a hemisphered submucosaltumor 10mm in diameter with a central pit in the descending colon and a similar lesion 6mm indiameter covered with nomal mucosa in the cecum. Intraluminal ultrasonography with a 20MHz micro-scanner revealed a mass in the decending colon having heterogenous hypoechoic pattern with patchy internal hyperechoic and anechoic areas in the 3rd layer of the wall, and the other mass in the cecum showing a homogenous hypoechoic pettern in the 3rd layer of the wall. Based on the findings, endoscopic resection was successfully performed. Histological examination showed that the tumor in the descending colon was glomus tumor in the submucosal layer, and the other in the cecum was neurofibroma in the same layer.

    We encountered five cases of colorectal inflammatory myoglandular (IMG) polyps. Since Nakamura et al. reported in 1992, 43 cases of IMG polyps have been reported, We reviewed 48 cases including our five cases clinicopathologically and elldoscopically. There were 34 men and 14 women with ages rallging from 15 to 78 years(average 51). Their main symptoms were fecal occult blood or bloody stool (25 cases) but eight patients had no symptoms. Except for one cases of terminal ileum, the other polyps located from rectum to transverse colon, and all polyps were solitary. A number of polyps measured over 10 mm in diarneter. Endoscopically, the shapes were pedunculated;the surfaces were smooth, reddish, partially with erosion, and sometimes lobulated. There were no malignant cases.

    A 74-year-old male under treatment of myasthenia gravis visited to our hospital with the complaint of frequent passage of soft stool. He was treated with predonisolone and zathioprine for over 4 monthes. Barium enema x-ray study before treatment clearly demonstrated en-face niche arlddeformities of the ileocecal region. Endoscopic examination before treatment showed the excavated ulcer and insufficiency of the ileocecal valve. Histological findings of biopsy specimen showed the trophozoite of Entamoeba histolytica. Based on these findings, the patientwas diagnosed as having amoebic colitis. Symptoms disappeared after treatment with metronidazole for 20 days. Barium enema x-ray study and endoscopic examination were performed after therapy. The ulcers were improved and Entamoeba histolytica were not detected from the biopsy specimens of the ulcer. This case suggested that immunocompromised hosts with soft stool must be excluded from the infection of Entamoeba histolytica.

    For palliation of colQrlic obstruction, we placed expandable metal stents in three patientswith transverse colonic carcinoma. Two patients started to eat several days after the procedure, and could be discharged. One patient died of acute cardiac failure caused by miocarditisfive days after it. Although stent placement in the transverse colon has been reported to bedifficult, making the intestine shorten and the delivery system lubricate, and elongating thedelivery systern facilitated the stent placement.

    A 59-year-old woman was admitted to our hospital with complaints of poor appetite ancbody weight lass during treatment of rheumatoid arthritis. Laboratory tests revealed elevatiorof tumor markers, CA19-9 was 1, 075U/ml, and DUPAN-II 595U/m1. Abdorninal US and CTshaved enlargement of pancreas. ERP revealed a diffuse irregular narrowing of the mainpancreatic duct.DIC showed smooth stenosis of terminal common bile duct. After steroidtherapy, the narrowing of the pancreatic duct and the stenosis of the bile duct were improved.Serurn levels of CA19-9 and DUPAN-II also decreased to the normal range.

    [Background]We have encountered some complications of the endoscopic sphincterotomy(EST)that are caused by the high frequency generator. we used a new high frequency current(Icc200, Erbe, Germany)for EsT from Novernber 1998. The Icc200 automatically controls it'soutput as tissue resistance varies.[Methods]We compared the ICC200 and a conventionalgenerator(UES10, 01ympus Optical Co.)with respect to the complication rate after EST. In 112patients frflm November 1998 to October 1999. There were no significant differences of sex, age, and papillotom between them. [Results]No perforation occured after EST. There wasin mild pancreatitis 1.6%and 3.6%respectively, while, 1.8%and 5.4%required ethanolinjection for hemorrhage, respectively. There was no hemorrhage in 53.6%and 35.7%of thepatients, respectively. The complication rate after EST using the ICC200 was lower than thatwith UES10.[Discu.ssion]The ICC200 was safer for EST than the conventional generator.

    To investigate the illdicationand significance of the laparoscopy, we surveyed ten hospitalsby questionnaire, where laparoscopy is routinely done, in Ehime prefecture. The number of patients who received laparoscopy was maxir:num in 1995, and then decreased sharply. The mderlying reasons are 1)the decrease of number of patients with chrQnichepatitis type C, 2)the development of other non-invasive exarninations, 3)fillancial problemof hospital management. The diseases in which laparoscopy is well indicated are prirnarybiliary cirrhosis and autoimmune hepatitis, metabolic liver disease, and cryptogenic liverdisease. Blind biopsies are nOt always helpful for diagnosis of these diseases, because the degrees Qfthe pathological change within the liver are not unLiform i:n these diseases. However, biopsiesfollowing laparoscopy are useful because liver biopsy specirnens can be obtained from areawhich shows particular findings.
