    Generally speaking, the clincian has traditionally used 5-year rate for comparing two groups of cancer treatment.
    However, there were no significant difference between two groups of lung cancer at 5-year rate sometimes due to these poorly results, even though there were significant difference in whole survival-time patterns between two groups.
    We had three numerical factors to consider that is to say difference, significance, and power testing significant difference between two groups of cancer treatment in statistics, and done test for significant difference between two groups as following data using the Chi-square test described by Mantel & Haenszel.
    1) Comparing Koga's numerical example that survival of 106 patients of lung cancer treated by Tele Cobalt 60 radiation therapy with survival of 145 patients of lung cancer treated by similar therapy reporting by Yamashita.
    2) Comparing Kuttig's numerical example that survival of 916 patients of lung cancer treated by Tele Cobalt of 60 radiation therapy with survival of 177 patients of lung cancer treated conventional X-ray therapy reporting by Kohler.
    3) Comparison between tumor size and survival time of 188 patients of lung cancer by resection reporting by Ishikawa.

    1. The death rate of lung cancer in Yokkaichi district were investigated for sixteen years between 1957 and 1972. The coarse death rate per 100, 000 persons increased in the polluted zone as well as the unpolluted zone till 1964, however has increased more in the polluted zone since 1965 except 1970.
    2. Thirty cases of fifty patients who died of primary lung cancer at the Yokkaichi Municipal Hospital for the six years between 1967 and 1972 were dissected. And the histological types of the thirty cases were classified into 12 adenocarcinomas, 6 epidermoid carcinomas, 10 undifferentiated carcinomas and 2 others.
    3. The five cases that were recognized as sufferers by air pollution died of lung cancer and were three males and two females from 66 to 73 years old. The diseases for which the five cases had been recognized were 3 chronic bronchitis, 1 bronchial asthma and 1 chronic bronchitis with bronchial asthma and chronic emphysema.
    Brinkmanns' index of 3 males in their smoking history were over 600 and no smoker was only one female.
    The histological diagnosis of the four dissected cases were 1 adenocarcinoma, 1epidermoid carcinoma, 1 undifferentiated carcinoma and 1 adenoid cystic carcinoma.
    The other one who was not dissected was undifferentiated carcinoma according to the biopsy of supraclavicular lymph node.
    As to the time of onset, the relation between lung cancer and recognized diseases was as follows: Concerning three cases, it may be at the same time or soon after the other.
    And concerning two cases, evidently the latter were prior to the former.
    4. Further investigation and observation is indispensable in order to analyse the correlation between lung cancer and air pollution in Yokkaichi.

    Cytologic examination was made of 10 cases of giant cell carcinoma of the lung confirmed histologically and electron-microscopically. Seven cases with a central type tumor showed positive cells in sputum. Three cases with a peripheral type tumor showed no positive cells in sputum and specimens obtained by the TV-brushing method.
    Exfoliated cancer cells of giant cell carcinoma in sputum, bronchial smear and pleural effusion showed an uneven dispersed distribution or a loose cobblestone-like arrangement. No marked anisokaryosis was observed. Occasionally tumor cells were found in nectrotic substances. The tumor cells were round or polygonal in shape, the cytoplasm was abundant and contained some vacuoles of different sizes, the nuclei of tumor cells were round or polygonal, the nuclear borders were smooth, chromatin was finely or coarsely granular, the nucleoli were large or small. Occasionally, cannibalism or the presence of multinucleated giant cells was seen in smear.

    A patient, 35 yio female, with pulmonary metastasis of choriocarcinoma occurring 4 years after the simple total hysterectomy for choriocarcinoma is the case.
    In spite of some clinical evidence of pulmonary metastasis, no elevation of urinary HCG level was detected. Slight elevation of HCG was observed only 5 months after the recognition of an abnormal shadow on chest X-ray.
    Improvement was seen on the administration of MTX and VLB, which was followed by the occurrence of tolerance to the drugs. Upon the trial with VCR, on which very few experience has been reported, a definite response, if not dramatic, was observed. Two months later the patient died. Autopsy revealed the extensive metastases of choriocarcionoma.
    Significance of measurement of low titer of HCG and trial with VCR in the case of choriocarcinoma were discussed.

    A 57 year old male came to our clinic with the chief complaint of general fatigue. Radiologicaly an abnormal shadow 2.5×2 cm. in size was found at the middle area of the right lung.
    During right thoracotomy, many swollen lymph nodes existed in the mediastinum. One of them was biopsied for pathological examination, which revealed non casiating epithelioid granuloma with metastasis of cancer cells. In 6 months after the operation, a right subclavicular lymph node has been palpable, which was diagnosed as the metastatic lymph node.
    The patient was died of general metastasis of cancer cell. At autopsy, the tumor was squamous cell carcinoma and there was no lymph node with sarcoid-like reaction.
    On the basis of above mentioned findings, the relationship between cancer and sarcoid-like reaction was discussed.
