    Thirty-five patients with small cell carcinoma of the lung were treated at the Departinent of Medicine, Okayama University Hospital during the period of January 1968 and March 1977. Respdnse rate was higher in polychemotherapy than in monochemotherapy. Best result was obtained by combintaion with cyclophosphamide, vincristine, methotrexate and procarbazine (COMP). Eleven of 14 patients responded to COMP therapy: 3 patients (21%) had complete clinical remission of disease and 8 (57%) had a minimum of 50% reduction of tumor mass. Better survival rate was obtained by a combination of intensive chemotherapy and radiotherapy than with radiotherapy alone or chemotherapy alone. However, no improvement in survival was noted with combination of chemotherapy and radiotherapy when chemotherapy yielded no clinical response. The median survival time for complete responders was 12+ months (range: 7.5-53+, mean: 18.4), 8.4 months (range: 2.5 12, mean: 7.8) for partial responders and 4.3 months (range: 1-8, mean: 4.2) for nonresponders. The difference between responders and non-responders was statistically significant.

  • Study on the correlation between Inhalation-washout and perfusion-washout

    For the purpose of clarifying the relationship between pulmonary ventilation, perfusion and their washout in various pulmonary diseases, pulmonary scintigraphy using 133Xe gas and 133Xe solution was performed in thirty two patients including eighteen patients with lung cancer.
    Two washout indicies, early and late phase, were obtained by using a technique of digital computer simulation.
    Following, results were obtained.
    1) There was good correlation between early washout indices of the total lung and F.E.V.1.0% in slightly to moderately impaired lung especially after infusion. Late washout indices of the total lung was well correlated with % V.C. as well.
    2) In lung cancer, there were highly significant correlation between relative inhalation counts and relative perfusion counts in impaired region. The perfusion was more severely impaired than the inhalation.
    3) In impaired region, early washout rate of the infused was slower than that of the inhaled. There was good correlation between late washout rate after infusion and that after inhalation.
    4) No correlation existed between the distribution and the washout rate both in inhalation study and perfusion study.
    5) Relative inhalation and perfusion counts per washout indices were useful in distinguishing impaired region from normal region.

    This paper described a statistical analysis concerning the routes of dissemination of cancer of the lung based on 1, 336 autopsy records, including three histological types; squamous cell carcinoma, adenocarcinoma and anaplastic carcinoma.
    The records were quoted from the annuals of pathological autopsy cases in Japan in 1972.
    The analysis of average number of cases with metastases at minor metastatic sites was carried out using occurrence or non-occurrence of major metastatic sites (lung, liver and bone) as 2 x 2 x 2 factorial design.
    The results were as follows; 1) The dissemination of squamous cell carcinoma of the lung is influenced by the presence of metastases in liver. 2) The dissemination of anaplastic carcinoma of the lung seems to occur a “One-step process” rather than occur a “Multi-step process”.

    A 65-year-old woman with chief complaints of anorexia, nausea and epigastralgia was diagnosed as primary lung cancer. She was suffered from dysphagia, dysarthria, hypesthesia of superficial sensation of left limb, left-hemiparesis and cough after admission. The cancer originated from the right upper lobe (Si) of lung metastasized to the stomach, the right tonsil, the epiglottis, the spleen, the pancreas, the right hemisphere of the brain, hilar lymph nodes, periaortic lymph nodes and the bilateral adrenals. Histological examination revealed that the cancer was undifferenciated large cell carcinoma including considerable number of giant cells, cannivalism and positive Alcian-Blue PAS stainning.

    A case of 27 yeaf-old-female with pulmonary lymphosarcoma was reported. She had no symptom, but a mass shadow was found on the left middle field of the routine chest X-ray, for which she was admitted and was operated upon. On admission the bronchogram revealed slightly narrowed left B4 and B5 and the Ga scintigram showed an area of abnormally high up-take. Operatively the mass was located at the lower portion of the left upper lobe and the boundary of its growth was not distinct. There was no evidence of the similar mass outside the upper lobe, and left upper lobectomy was performed. The histologic examination of the resected mass revealed malignancy of non-epithelial cell, which was diagnosed as primary lymphosarcoma of the lung. Four years have passed Since the operation and the patient has been well.

    A 54-year-old man with histologically proved pleural diffuse mesothelioma was presented. The patient was treated with intrapleural administration of normal allogeneic lymphocytes (3-5 x 108 cells) and BCG-CWS in combination with a multiple drug regimens consisting of AVF (ACNU 2-2.5mg/kg on day 1, vincristine 0.02mg/kg FT-207 16mg/kg, on day 1, 8 and 15, respectively) therapy on a 6 week cycle, which was associated by stabilization of the chest film, no pleural effusion and improvement in pain. However, he relapsed 3 weeks after 2 cycles of AVF therapy. Adriamycin (1.2-1.5mg/kg, i. v. every 3-4 weeks) was restarted in combination with intrapleural administration of BCG-CWS, and a marked improvement on the chest X-ray with no pleural effusion was obtained. However, immunological monitoring showed impaired immune reactivity during the clinical course. Adriamycin and BCG-CWS administrations were stopped after a cumulative dose of 380mg and 5.4mg respectively. He is in remission state in excess of 16 months from the onset time of symptoms.
    This regimen of intrapleural BCG-CWS administration in combination with chemotherapy reems to have a worthwhile antitumor effect and thereby stabilize diffuse pleural mesothelioma.

    Case records of histologically proven 2, 493 patients with lung cancer were submitted for analysis for the purpose of the establishment of TNM staging on a basis of a prospective field study which was carried out cooperatively at 87 participating institutions in Japan during the period 1972 to 1974.
    JIC·ETNM Classification (1978) was established as results of an analysis of 2, 493 cases by the existing TNM staging. As to the ranking of T1 N1 M0, wheather it would be in stage I or in stage II, our conclusion was that it would be justificable to put it into stage II.
    Expansive coincidence between JJC·ETNM classification (1978) and Japanese Clinical Staging System was ascertained principally.
    Comparison between JIC TNM classification (1978) and UICC TNM (1978) classification was tried.

    From case records of 2, 493 patients with lung cancer which were carried out cooperatively at 87 institutions in Japan during 1972 to 1974, some clinical statistical data were obtained. Prognosis of female on stage I in Japanese Clinical Staging is significanly better than male. Prognosis of under 64-year-old group is significantly better than over 65 year-old group. Prognosis of epidermoid cancer group is not significantly better than adenocancer group. Prognosis of epidermoid-and adenocancer group is significantly better than small cell cancer group. Prognosis of curative operation group is significantly better than any other therapeutic methods.

    Records and classifications of the site of lymph nodes to confirm metastatic site (lymph node mapping) have not been officially established for the lung cancer study in Japan.
    In the National Cancer Center, lymph node mapping according to metastatic site has been studied by means of histopathological examination of the hilar and mediastinal lymph nodes in resected lung cancer cases. The lymph node mapping had already been accepted in the American Joint Committee.
    The proposal to establish proper lymph node mapping of the lung was carried with acclamation in a council of the 18th Japan Lung Cancer Society.
    As an initial step, private meeting concerning with lymph node mapping was held in the National Cancer Center. In this meeting, trial use of modified lymph node mapping bassed on the National Cancer Center was supported.
    The establishment of standard form of lymph node mapping in the Japan Lung Cancer Society after trial use of this mapping will be expected.
