    30 patients of unresectable primary lung cancer were treated with polychemotherapy and small dose irradiation concurrently. Intermittent small dose irradiation was used onpurpose to sensitize the tumor cells to the effect of the chemotherapy. Patients were irradiated 200 rads on day 1, immediately before the intravenous administration of anticancer agents. (for example, Cyclophosphamide 20 mg./kg., Vincristine l mg., 5-Fluorouracil 250mg. in small cell carcinoma.) From day 2 to day 7, patients were given 250mg. of 5-Fluorouracil intravenously. Therapy was continued for 4 weeks, and the effect was evaluated on chest X-ray films. Regression of tumor size more than 50% calculated by product of two perpendicular diameters was defined as an objective response.
    Overall response rate was 53%. The response rate in relation to cell type was, 100% in small cell carcinoma (10/10), 67% in large cell carcinoma (2/3), 33% in epidermoid carcinoma (2/6), and 18% in adenocarcinoma (2/1 1).
    No remarkable side effect to the addition of small dose irradiation was observed.

    Cell-mediated immunity was investigated serially in 60 patients with primary lung cancer in association with disease state and prognosis. The blastogenesis of peripheral blood lymphocytes in response to PHA, ConA, PWM was investigated to evaluate the cell-mediated immunity.
    The response of these mitogens was almostly correlated with various clinical performance status.
    The responses of these mitogens was sensitively affected by a variety of factors related to therapies. Operative therapy induced immunosuppresive state but immunotherapy recover from immunosuppresive state. The depressed blastogenetic responses in patients with curative and relative-curative operation returned to the lebel around the preoperative status after starting immunochemotherapy. In two months latter after the operation, the blastogenetic responses was down to the operative level and gradually recovered to the level of normal patients. The changes in mitogenic responses could be termed so-called “Rebound phenomenon”. But this phenomenon was not detected in patients with non-curative operation and their responses remainded throughout the observation.
    From these studies, preoperative blastogenetic responses may be correlated with clinical features, such as the ability to recover from immunosuppresive status caused by the operation or the reliable clue to predict a prognosis.

    A total of 44 patients (36 bronchogenic carcinoma and 8 metastatic lung tumor) were treated with ifosfamide in dose of 150-200mg/kg, fractionated to 3-5 consecutive days, every 3 weeks. All patients had disseminated conditions, and 21 had previous chemotherapy. Bladder toxicity was minimized by oral and/or parenteral high fluid intake, and bladder irrigation was not utilized. In bronchogenic carcinoma patients (8 adeno, 1 large cell, 1 alveolar, 7 squamous and 19 small cell), 8 of 32 evaluable patients responded positively with the effect lasting 1.5 to 8 months (median, 4 months). Seven responders (1 complete and 2 parital) had small cell carcinoma and one partial responder had adenocarcinoma. All of the 3 cervical carcinoma patients with pulmonary metastases responded to the therapy (1 complete and 2 partial), but no positive response was observed in 5 patients with metastatic lung tumor (originated from ovarian carcinoma, endometorial carcinoma, malignant neurinoma, hemangiosarcoma and rhabdomyosarcoma). Myelosuppressive toxicity was mild: leukopenia (less than 2, 000/cmm) occurred in 12% and thrombocytopenia (less than 100, 000/cmm) in 10% of patients. Bladder toxicity occurred in 51% of patients (macrohematuria constituted 17%). Upper gastrointestinal symptoms, such as nausea and vomitingoccurred in one-half of patients. The overall response rate was 28%(11/40 evaluable patients). In conclusion, ifosfamide appears effective in the management of disseminated cervical carcinoma, as well as small cell lung carcinoma, and should be considered in combination therapy with other agents.

    19790417Five autopsy cases of bronchiolo-alveolar carcinoma seen between 1973 and 1977 were studied in the point of clinico-pathological correlations. Histological findings of three patients with large amounts of sputum, no metastatic manifestations, and bronchogenic spread of infiltrative tumors in chest roentgenogram, were consisted of uniform tall columnar cell with mucin secretion. In these cases, tumor cells did not invade blood and lymphatic vessels, and extrathoracic metastases seen rarely. On the other hand, two cases of bronchiolo-alveolar carcinoma consisted of low columnar or cuboidal cells showed early extrathoracic metastasis clinically and pathologically. The rout of intra-pulmonary metastasis in these cases seemed to be hematogeneous because of simultaneous and uniformly dissemination of multiple nodular lesions in chest roentgenogram.

    Postoperative survival rates in 393 patients with adenocarcinoma of the lung were studied according to histological subtype, the grade of differentiation, clinical stage, size of tumor and the mode of operation.
    Regardless of histological subtypes better survival rates were achieved in the cases with well-differentiated adenocarcinoma. Stage I cases and curatively operated cases also showed good prognosis. There was no difference in post-operative survival rates between papillary type and tubular-cribriform type of adenocarcinoma, irrespective of the grade of morphological differentiation. This suggests the necessity of a new device for subtyping adenocarcinoma of the lung.
    Combination chemotherapy with cyclophosphamide, mitomycin C and 5 -fluorouracil was found to be the best postoperative adjuvant chemotherapy in this study.

    An autopsy case of 31 year old man with primary pulmonary leiomyosarcoma was reported with a discussion on the various nature of this tumor. The primary tumor (3.8×2.6×2.0 cm in diameter) arising from the peripheral region of the left lower lobe (S9), was well circumscribed, and yellowish-white, soft and friable. Microscopically, the tumor was composed of aggrigation of spindle shaped cells arranged in interlacing bundle. Fineststructure of the tumor displayed numerous microfibrils with dense patch in the cytoplasm. Partial resection of the left lower lobe was performed. However, the patient died 3 years and a half after operation due to the recurrence of the tumor. At autopsy, the recurrent sarcoma formed a huge tumor involving left thoracic cavity, subcutaneous tissue and upper abdominal cavity but no metastasis to other organs was observed. To our knowledge, present case is the 37th of primary leiomyosarcoma of the lung in Japanese literature.

    A 57 year-old male, who had a small squamous cell carcinoma at right main bronchus, was operated. However, a curative operation was not performed, because the tumor was not recognizable intraoperatively. He died after 8 years postoperatively. The autopsy revealed very wide intramucosal development, i.e. to cricoid cartilage and left main bronchus. Lymphnode and hematogenous metastases were minimal. This case may represent the clinical course of the superficial spreading type of squamous cell carcinoma from the stem bronchus, that is, slow growing, minimal metastasis andmucosal involvement in wide area.

    A sixty-six year-old chromate worker with multicentric lung cancer was repori ed. There were two separated epidermoid carcinomas in the left lung, one from the upper lobe bronchus and the other from the lower lobe bronchus. His left lung was resected and the specimen was studied by the messurement of concentration and the X-ray microanalysis of chromium. Generally, the higher concentration of chromium was observed in the tissues with abundant anthracosis. The chromium was mainly composed of Cr +3. By X-ray microanalysis, particles derived from inhaled chromium dust were foundespecially in the tissues with anthracosis.

    A 29 years old female with benign clear cell tumor of the lung, which was first reported by Liebow & Castleman, was presented. She was accidentally found to have a coin lesion in the left lower lung field at the chest X-ray examination.
    The tumor was easily enucleated from pulmorary parenchyma. Grossly, the tumorwas spherical, measuring 2.9×1.8×2.0 cm and well demarcated. There was no evidence of involvement of a bronchus or major vessels.
    Microscopically, the tumor was composed of large, clear polygonal cells with glycogen granules and many thin walled vessels. The Reticulum stain showed non-epithelialpattern.
