    The use of antineoplastic drugs in cancer patients is often limited by their side effects, especially leukocytopenia. OK-432, a preparation from a low virulent strain of hemolytic streptococcus has been studied for its effect to stimulate the proliferation of hematopoietic cells, induce the release of colony stimulating factor and increase peripheral white blood cell count (WBC). The present study was undertaken to know whether OK-432 can cause remission of the leukocytopenia induced by repeated administration of antineoplastic drugs. OK-432 (2KE-5KE) was intracutaneously given to patients with lung cancer every 2 or 3 days commencing immediately after the second course of chemotherapy. The WBC and polymorphonuclear leukocyte count (PMN) nadirs after the second and the third chemotherapy courses were significantly lowered compared with the nadir after the first chemotherapy course in cases which were not given OK-432. On the contrary, no such differences in nadirs was observed in cases receiving OK-432 administration, showing that the decrease of leukocyte counts following chemotherapy were prevented by OK-432. Our results indicate that use of OK-432 is recommended to reduce the risk of infections in cancer patients given chemotherapy.

    The presence of keratin positivity in tumor cells and infiltration of Langerhans cells (LC) in tumor nests and in areas of adjacent lymphocytic infiltration was investigated by ABC immunoperoxidase methods using anti-whole keratin antibody and anti-bovine S-100 protein antibody in primary (50 cases) and metastatic (5 cases) lung carcinomas. A large number of S-100 (+) dendritic cells (DC) were present in 13 out of 14 cases of bronchiolo-alveolar carcinomas which were well differentiated adenocarcinomas (ADC). Tumor cells of BAC which resembled epithelial cells of the airway were constantly positive to anti-KR antibody. Furthermore, positive KR reaction of tumor cells correlated well with the presence of S-100 (+) DC in tumor nests. Tumor cells of other lung carcinoma types showed no such relationship evidence, and S-100 (+) DC were present only in less than 30% of these cases. Irrespective of histological types of tumors, S-100(+) DC were present in adjacent lymphocytic infiltration. The prognosis of BAC is generally better than that of other ADC, because LC have the ability of antigen presentation. These S-100 (+) DC may play a role in tumor immunity.

    We report a case of metastatic osteosarcoma which showed prominent accumulation of radioisotope on bone scintigraphy and was treated with extended resection. The patient is a 44 year-old male who underwent amputation above the knee for osteogenic sarcoma of the left thigh when he was 38 years old. Four years later, well-circumscribed tumors in the left lung field and the nearby chest wall were detected on the plain chest roentgenogram. There was prominent accumulation of the radioisotope on the bone scintigram over the same area. Left lower lobectomy with partial resection of the adjacent chest wall was carried out. Pathological examination confirmed the resected tumor to be metastatic from the osteogenic sarcoma. Two years later, a huge recurrence in and around the left thoracic cage was recognized. Strong hot spots were also evidenced on the bone scintigram. The metastatic tumor involved the left chest wall, mediastinum, diaphragm, and the retroperitoneum. Tumor was extirpated with completion pneumonectomy, total resection of the parietal pleura, extended resection of the left chest wall (8 ribs were resected), wide resection of the pericardium and subtotal resection of the left hemidiaphragm. Two years after this procedure, the patient is well-rehabilitated.

    A 52-year-old man with malignant thymoma presenting S.V.C. syndrome received cisplatin-prednisolone combined chemotherapy preoperatively. As a result, disappearance of the syndrome and remarkable diminution of the tumor were obtained in a short period of time, making circulatory reconstruction unnecessary and total resection possible. Furthermore, postoperative irradiation (50 Gy) and prophylactic readministration of cisplatin were performed. This multi-disciplinary treatment has resulted in 9 months postoperative survival without signs of reccurrence.

    The patient was 66-year-old female. She was referred for evaluation of an abnormal shadow in the left S6 on the chest radiograph. The disease was subsequently treated as a tuberculoma. Since the abnormal shadow in the left S6 increased gradually, she was referred to our hospital in August 1986. Bronchoscopy detected a tumor in the left B6 which was diagnosed as mucinous carcinoma. She had a medical history of radical operation for breast cancer in 1963. The finding that both were one and the same mucinous carcinoma led to the diagnosis of breast cancer metastatic to the lung. The cancer was observed to be isolated to one lung; therefore left lower sleeve lobectomy was performed on September 11. This was an unusual case of lung cancer metastatic from breast cancer treated surgically 24 years previously.

    A case of roentgenographically occult triple lung cancer was reported. The case, a 67-year-old-male, was initially detected as a result of a sputum mass survey in March 1982. The first tumor was located in left B1+2a+b, the second tumor was detected in left B6 by follow-up bronchoscopy in March 1987, and the third tumor in right B2ab was also detected by follow-up bronchoscopy in September 1987. Photodynamic therapy (PDT) was performed, the tumors completely disappeared clinically, and he is at present apparently disease-free. Using the criteria suggested by Martini et al., we decided this case was multiple primary lung cancer. Patients with multiple primary lung cancer have a good chance of surviving for a long time if the tumor is detected at an early stage, and it is emphasized that follow-up after therapy is very important.

    A 64-year-old male case of lung cancer producing α-fetoprotein (AFP) was reported. Chest X-ray film revealed a large round mass lesion in the right upper lung field. Percutaneous lung biopsy revealed poorly differentiated adenocarcinoma. Although the serum concentration of AFP was 20, 500 ng/ml, there was no evidence of tumor mass in the liver or other digestive organs by CT and ultrasonography. Right upper lobectomy after radiation therapy was performed. It was strongly suggested that the lung tumor produced AFP because the serum AFP level decreased with treatment. Production of AFP by lung cancer cell was confirmed by an immunoenzyme labelling technique using PAP. There have been only ten cases of production of AFP by lung cancer cells in Japan.

    A 56-year-old male was admitted because of complete atelectasis of the left lung caused by tumor. The tumor occluding the left main bronchus was squamous cell carcinoma. After bronchial artery infusion (Mitomycin C: 10mg), left pneumonectomy with lymph-node dissection was performed. Macroscopically the tumor had almost disappeared leaving a lesion sized 1.0×0.7×0.6 cm below the orifice of the lower lobe bronchus. Histological examination revealed no tumor invasion beyond the bronchial wall and no metastasis to lymph nodes. This case was an early stage hilar lung cancer. Bronchoplasty would have been a better selection for this case instead of pneumonectomy. After operation the patient said that he had expectorated a piece of tumor before operation. Preoperative application of endoscopic Nd-YAG laser is sometimes recommended for polypoid hilar lung cancer. It could enable more flexible planning of surgery or prevent tumor aspiration into the airway during operation.
