Author has undertaken following experiment in order to investigate the effect of Streptomycin (SM) to leprosy. The subjects consist of 48 individuals: 45 lepromatous, 1 neural and 2 tuberculoid cases.
These subjects were deviled into two groups
(a) 20 cases of exclusive SM administration
(b) 2B cases of duplicate administration of SM with promin
The ways of administration were as follows: i. e. The day dose of SM was 1.0g intramusculary with maximum duration of 7 months ; the maxmum total dose was 150g and the minimum 40g and the results are summarized as follows
(1) The resorption of leproma and infiltration were 38% in group (a) and 85% iungroup (b).
(2) Improvement of nasal stoppages and ulcer were 57% in group (a) and 95% in group (b).
(3) Neuro-muscular symptoms (disturbance of both motility and sensory) showed almost no change.
(4) Appearance of erythema nodosum leprosum was observed 11 % in group (a) and 37% in group (b).
(5) Mitsuda reaction showed no change.
(6) Rate of sedimentation of RBC improved 44%, staitionary 28%, aggravated 28%.
(7) Bacteriological examination of skin, diminution of bacilli in number or degeneration of morphology: 17% in group (a).
(8) Liver function test showed no change.
(9) Complication : slight dizziness in 6 cases.
(10) Result in total
Group (a) Group (b)
Improvement 45% 82%
Staitionary 45% 18%
Aggravation 10% 0
In the disease which demand a long period of treatment as leprosy, it is obviously unr-easonable to draw a conclusion basing upon the data of the short period treatments. In our experiment, observation was carried out throughout 7 months which led to the following conclusion:
1) In the case of exclusive SM administration, an improvement is discernible in a cert-ain extent, using the subtoxic-maximum dosis of SM.
To enhance this effect, it is therefore postulated to minimize the toxicity of drug and to increase the possibility of long period administration.
2) Rate of improvement was higher in the case of duplicate administration of SM and promin than the independent dosage of the latter, upon which no special stress should be laid in the present status.
3) Among those cases, which showed appreciable improvement by SM, no marked res-orption of the histo-pathological change was obtainable as was observed in promin treatment.
4) Neverthless even in the case of a aggravation of nodular and infiltrative cases during the promin treatment, duplicate dosage of SM brought a definite relieves in the resorption of foci.
