    In the intrinsic coloring technique of facial prostheses, it is necessary to laminate an appropriate quantity of silicone materials on the appropriate place. A thixotropic agent (TA) was applied to silicone materials to improve their high fluidity. The aim of this study was to evaluate the effects of the amount of TA on the fluidity of a maxillofacial silicone (A-2186) and two silicone materials (KRS-10-2, KRS-C2-2). Specimens for A-2186 and KRS-10-2 were prepared with different amounts of the corresponding TA: from 0 to 3 drops for 10 g of base silicone. 0.1 g of mixed silicone was placed on a glass plate and then the plate was positioned horizontally or vertically. The horizontal diameter and the vertical length of each specimen were measured until 15 minutes after the start of mixing. A load of 14.7 N was applied to the glass plate over 0.5 ml mixed silicone for the consistency test, and the diameter of silicone after 15 minutes was measured. All silicone materials without TA showed high fluidities, while all silicone materials mixed with more than 0.4 drops showed low or no fluidity. These results suggested that a small amount of thixotropic agent is effective to improve the fluidity.

    The aim of this study was to compare the mechanical behavior of two different sizes of maxilla with obturator prosthesis by the three-dimensional finite element method (3D FEM). 3D FEM models of an obturator prosthesis with Aramany class Ⅳ defect maxilla and a removable prosthesis with non-defect maxilla were constructed with mucosa, alveolar bone, periodontal ligament, 24, 25, 26, 27 teeth, and double Akers clasps. In the boundary condition, the base area of alveolar bone was fixed in all directions. A vertical force of 30 N was applied to the center on the equivalent of 14, 15, 16, 17 teeth (Total: 120 N). Maximum values of stress at the supporting bone in the Aramany class Ⅳ model were higher than those in the removable partial denture model. The direction and amount of displacement were different, and were related with bone size. A higher value of displacement was observed in the Aramany class Ⅳ defect model, and rotational motion was also observed. A higher stress value and wider stress distribution on the alveolar bone were observed in the Aramany class Ⅳ maxillary defect model. Rotational motion of the obturator prosthesis was observed in the Aramany class Ⅳ defect model.

    This case report describes the successful long-term prosthodontic treatment of a maxillectomy adolescent patient up to 7 years. The patient was a 12-year-old female who underwent right maxillary resection due to adenomatoid odontogenic tumor at Tokyo Medical and Dental University hospital. The patient was 15 years old at her first visit to our Maxillofacial Prosthetics clinic. There was no facial deformity, and the right maxillary molars and second premolar were lost by the tumor resection. The defect was reconstructed with buccal fatty tissue and there was no communication between the oral cavity and nasal cavity, however, the defect site was easily deformed by pressure. Because of tight teeth contact, we carefully analyzed the clearance of clasps and practiced teeth preparation on a study model before preparing the abutment teeth. In consideration of the patient’s esthetics and comfort, a metal based dento-maxillary prosthesis was fabricated. Occlusal contact and oral hygiene were checked, adjusted and maintained at regular intervals. Findings such as caries or broken clasps were absent throughout the 7-year follow-up.

    For color reproduction of an facial prosthesis, internal or external color is selected. The internal color is superior with regard to depth, permeability, and stability; however, it is difficult to reproduce. Colorimetric analysis of silicone facial prosthesis materials (KRS-10-2 and KRS-10F-2; GC) using newly developed colorants (CA-R, CA-Y, CA-B, CA- P1, CA-P2, CA-B1, and CA-B2) was performed using samples of varying thickness. There was little colorimetric difference between KRS-10-2 and KRS-10F-2 when silicones were over 5mm thick. Although the color differences among mixed-colored materials varied little regardless of thickness, single-colored materials exhibited significant differences with regard to silicone thickness, particularly CA-Y. Further, few changes in color differences were detected in each sample 1 year after the initial measurement.
