Purpose:We clarify factors that make it possible to identify in-home care for end-of-life cancer patients who are using visiting nursing stations.
Methods:The target patients were diagnosed as suffering from "end stage cancer" among those who used a visiting nursing station from April 2013 to March 2017. Data was gathered from the medical records, and the attributes of the target person, the last place of residence, the ADL / physical condition, social resources, etc. Χ² test with the presence or absence of care at home as the dependent variable was carried out and items having significant differences were extracted, and multiple logistic regression analysis was conducted to verify the factors related to home nursing visits. This research was approved by the Ethics Committee of the Faculty of Health and Medicine, Showa University.
Results:344 people(279 at home: 81.1% at home, 65 people at hospital or institution: 18.9%)were analyzed. Results of Χ² tests revealed it was the elderly, no ADL movement, diet, excretion, bathing, changing clothes, and conditioning assistance were found to be significantly related to home care necessity, using diapers, visiting medical treatment, home was a detached house and owned house, hope of home care. As a result of multiple logistic regression analysis, it must be there is hope of home care(odds ratio <OR> = 11.9[95% confidence interval 5.3 – 28.4]),There is using diapers (OR = 0.4 [0.2 – 0.9]),Housing is a detached house(OR = 5.3[1.9 – 14.2]),was associated with end-of-life care at home.
Conclusion:In this study, factors related to in-home care were clarified, and it was possible to show different kinds of nursing support, but further research and analysis is necessary in the future.
