Aspiration frequently occurs in the elderly. Most aspiration in the elderly does not contribute to neuro-,or muscle-degeneration of the pharynx and larynx but the sensory and transmission impairment of neurons in the brain. Not only medical approaches but also caring/nursing approaches on neurons related to aspiration are important in medication for pneumonia or prevention of aspiration. In particular, olfactory stimulation with black pepper oil improves the swallowing reflexible movement in the elderly with dysphagia. The integrated approach with medication such as an angiotensin-converting enzyme or a dopaminergic agent, oral care, meal temperature, spice with capsaicin and menthol, and olfactory stimulation with black pepper oil can prevent aspiration or aspiration pneumonia in the elderly.

Aromatherapy, which utilizes actively the effects of fragrance, is a complementary and alternative medicine for the purpose of the prevention and treatment of diseases and clinical symptoms using essential oils. Essential oils, which are extracted by distillation, expression and so on, include various molecules. Because essential oils possess various kinds of pharmacological activities, including anti-microbial, anti-inflammatory, analgesic, anti-depressant, anxiolytic and relaxation-inducing activities, aromatherapy is very useful in the treatment of various gynecological diseases, skin diseases, upper respiratory illness, psychosomatic diseases, and in pain control and stress management. In aromatherapy, various methods are employed, e.g., aromatic bathing, massage, inhalation and application to skin or mucosal membranes. Aromatherapy massage is the most effective in the induction of relaxation. Because aromatherapy is complementary, an ideal medicine could be realized in combination of it with modern medicine and other complementary therapies.

In this article, we describe an overview of characteristics of horticultural therapy and introduce our study on the effect of cultivation and utilization of herbs. Work of herb cultivation was shown to have some effect on a recovery from neuroses and uncomfortable mood in the research using general health questionnaire and profile of mood states. It was also suggested that horticultural work is effective on a recovery from stress and fatigue of the central nervous system by means of measuring the concentration of cortisol in the saliva and the critical flicker fusion frequency. We also report several cases which have been encountered during practice of horticultural therapy in a hospital and a welfare facility.
