    A number of substances for paracordal injection have been used in the treatment of unileteral vocal cord paralysis and so on.
    The chief reason for vocal disabilities in cases of unilateral vocal cord paralysis or hypogenesis vocalis with bowing is deficient glottal closure for phonation which may be successfully treated with paracordal injection.
    Recently new suitable materials were used and discussed, TEFLON by Arnold and SILASTIC (medical grade silicone) by Rubin.
    We reported an experience with SILASTIC injected into the unilateral vocal cord paralysis (14 cases) and atrophy of the vocal cord (12 cases).
    The result was estimated with phoniatric examinations, such as phonation time, the ratio of phonation time and expiration time, and sound spectrogram etc.

  • Morphological Changes Resulting from Inhalation of Sulfurous Anhydride and Red Clay Particles

    Using electron microscopy, tracheal epithelia of four guinea pigs were examined after insufflation of sulfurous anhydride or red clay particles. In cases of sulfurous anhydride insufflation, no morphological changes were found in any cell of tracheal epithelia. When red clay particles were given in a short time, mitochondria of ciliated cells were found decreased in number. However, this was not always observed. In the cases in which insufflation lasted for a long time, cytoplasmic membranes of tracheal epithelia were found penetrated by the particles, resulting a domage of the cell structures. Even in the reticular and collagenous fibrils, few red clay particles were found.

    Out of 235 patients who received esophagoscopic examinations in our department, 14 cases of cancer of the esophagus were found, The relation ship of chief complaints and symptoms on their first hospital visits to the prognosis of their diseases was studied. In 41 cases, 28 males and 13 females, 1) 28 patients had a chief complaint of disturbance of deglutition, 2) 8 patients had unusual sensation on deglutition, 3) 4 patients had a pain on swallowing, 4) One patient had a sensation of passing food in the esophagus.
    Among the 28 patients with a chief complaint of disturbance of deglutition 8 patients had the first hospital visit within one month after they had developed the disturbance, 10 patients in 1 to 2 month, 3 in 2 to3 month, 4 in5 to 6 month, 3 after 6 month. In 13 patients, however, the disturbance of deglutition had been preceded by the unusual sensation on swallowing.
    To detect malignancy of the esophagus in theearly stage, every individual should be routinely examined in aeregular checkup of the esophagus as well as the stomach, and the subject who complains of any kind of abnormality should thoroughly be investigated by means of dynamic study with TV fluoroscopy and esophagoscopy.

    The intraesophageal pressure with esophageal speech on laryngectomized patients at the time of phonation was measured to study the mechanism and capacity of esophgeal speech. The results are as follows:
    Intraesophageal pressure always marked the same pattern no matter what was spoke. The wave patterns are the most complicated in the good esophageal speakers.
    Maximum pressure at phonation is the lowest and swallowing pressure is the highest in the good speakers. The value of a ratio of the integral quantity by deglutition to that of phonations is the lowest in good speakers.
    The measurement of intraesophageal is not onlyuseful to learn the function of pseudoglottis, but to teach esophageal speech and to find out secondary changes at hypopharynx, pseudoglottis and lower esophagus.

    Experiences of twenty-two cases on reconstruction of cervical esophagus after pharyngolaryngoesophagectomy were reported. Most of these cases had received radiotherapy by telecobalt or supervoltage X-ray preoperatively.
    Tubed pedicle flaps were useful and assure even for heavily irradiated cases. The disadvantageous influence of irradiation for reconstruction was found not serious in the case of supervoltage X-ray, because of its skin sparing effect.

    Esophagofiberscopic examinations (Olympus' E.F.S.) have been carried out in 510 cases in our clinic for past two and half years. During the interval a remarkable improvement of the instrument has been noted. Some important points are explained briefly as follows.
    The air-flush, water-flush for cleaning the lens and the inspisated secretion of the esophagus can be done by pushing each button. Thus these actions are possible in one-hund (left hund).
    The top of the instrument can be bent to 90° up and down directions. The end of the esophagus, the anastomotic portion between the esophagus and the stomach or the jejunum, and areas of anastomosis after gastrectomy are more easily observable.
    When photographing, the camera can be attached to the eye-lens of the instrument more easily; and in using the T.T.L. system, the pictures are taken in the same condition.

    A case of spontaneous retropharyngeal abscess of a 2 year old male was reported. A possibility of “cardiac arrest” incident to the operation of retropharyngeal abscess was discussed.
    This case suggested that in some cases general anesthesia and or tracheotomy may have to be resorted to in the operation of retropharyngeal abscess.
