    Eighty eight patients with the hypopharyngeal cancer were treated in our clinic from January 1979, to December 1983. Fifty four patients who died until June 1985 were analyzed. The causes of death were original cancer (48 cases, 89%), additional malignancy (3 cases, 5.5%), intercurrent diseases (3 cases, 5.5%).
    Clinical stage had a great influence on the prognosis and patients with cervicallymph nodes involvement had a very poor prognosis. In order to improve survival rates, early diagonsis and early treatment are seemed to be inevitable.
    Local recurrence was much noted in the mesopharynx. Elective irradiation may be reasonable to those cases where recurrence in the mesopharynx and Rouvière lymph node is anticipated.
    Thyroidectomy with paratracheal node dissection at diseased side may be indispensable not only for the postcricoid cancer but also for the piriform sinus cancer.
    Multidisciplinary therapy including chemotherapy and immunotherapy was seemed to be important, since distant metastasis was found 25% of patients died of original disease.

    Flow rates at low and high intensities of the voice were measured for 94 patients with various voice disorders. The usefulness of this measurement as a phonatory function test was studied in comparison with the simple measurement of flow rate during easy phonation.
    1) In cases of high stiffness of the vocal cords such as laryngeal cancer and laryngitis, the flow rate tended to be abnormally high at a high intensity of the voice while normal flow rate at a low intensity. For those cases, the measurment of flow rates at low and high intensities of the voice was more useful than the simple measurement of flow rate.
    2) Some cases of laryngeal nodules and polypoid vocal cords showed an abnormally high flow rate at a low intensity of the voice while normal flow rate at a high intensity.
    3) An expiratory insufficiency during phonation, one of the functional voice disorders, was diagnosed by simultaneous measurement of flow rate and vocal intensity, as such a patient was unable to phonate a strong voice while the flow rate remained within normal range at a low intensity of the voice.

    From 1976 to 1985, many professional singers visited our clinic and were phonosurgically and conservatively treated. A follow-up study with statistical and histopathological methods was carried out on 212 cases with complete clinical data, selected from all the cases.
    The results obtained were as follows:
    (1) The vocal polyp was more prevalent in male than in female.
    (2) The vocal nodules were prevalent in female singers.
    (3) Professional singers had a tendency to suffer from vocal nodules more than average people.
    (4) The histopathological study revealed that an edematous change in the submucosa was most dominant among many kinds of histological changes in female patients who are tend to suffer from vocal nodules, while hemorrhagic and infectious changes were found in addition to edematous ones in male patients who are tend to suffer from vocal polyps.
    (5) In 97% of the patients who were laryngomicrosurgically treated, normal restoration of the voice was obtained.
    From the results above mentioned, it would be emphasized that surgical resection of lesions under laryngomicroscopy could be carried out even for professional singers.

    The β-blockade theory is one of the hypotheses on the trigger mechanism of bronchial asthma. Although many reports have discussed whether or not asthmatic patients are in the β-blockade condition, the answer has not been obtained yet. In the present study, human peripheral lymphocytes were used for the experiment because they have the same β-adrenoceptor sub-type (β2-predominant.) of trachea and bronchi at where asthmatic response develops.
    We measured β-receptors, cyclic nucleotides and adenylate cyclase activity of the peripheral lymphocytes in asthmatics and controls. The patients were divided into four groups according to the kinds of drug used and one non-treated group. Effect of drugs on the above-stated factors was evaluated.
    In the non-treated group, β-receptor Bmax tended to be lower and lymphocyte cAMP as well as lymphocyte adenylate cyclase activity was lower in compared with those of the control group. This observation suggests that asthmatic patients were in the β-blockade condition. However, in the xanthine treated group in which all patients suffered from a mild asthma and required the drug, Bmax tended to be lower than the non-treated group. It has been reported that 15-HPETE, a metabolite of arachidonic acid, reduces Bmax. Further comprehensive and detail studies with metabolites of arachidonic acid and chemical mediators may be needed in order to answer the question whether the β-blockade condition is primary or secondary.

    Mucociliary transport protects the respiratory tract against inhaled particles, including bacteria, by clearing them trapped in mucus towards the pharynx. We have studied the effects of products of H. influenzae on frog ciliated epithelia using a method for determing mucociliary transport force. Bacterial products were prepared to fraction A and fraction B from culture fluid of H. influenzae. A product in fraction A depressed mucociliary transport in a dose dependent manner and it was thought to be protein. Fraction B stimulated mucociliary transport but its effect was diminished at a higher concentration. These results indicate that H. influenzae products may be important early in the infectious process by rendering mucociliary transport less efficient, allowing the organism to multiply in the respiratory tract.

    Nine cases with free jejunal reconstruction after hypopharyngolaryngoesophagectomy were evaluated reconstructed esophageal functions by manometric studies.
    The pharyngoesophageal high pressure zone disappeared, and the intra luminal pressure of freely transplanted jejunum was almost equal to atomospheric pressure, although the remaining intact esophagus including the lower esophageal high pressure zone had no significant changes on the resting pressure.
    Spontaneous movement of transplanted jejunum was observed for 26% of the examination time of four to five hours after meals. Movement similar to phase III intestinal contractions of Code's classification was seen intermittently for 43% of the spontaneous movement. Over night continuous recording showed that these rhythmic high pressure contractions were less predominant in the three hours after meals than in the four to twelve hours after meals.
    No propagating pressure waves were observed in the transplanted jejunum on deglutition with or without jejunal spontaneous movement, though synchronous spike waves were also seen in the pharynx and jejunum. However normal peristaltic waves in the residual esophagus and normal reluxation of lower esophageal sphincter also appeared.
    We speculate from this study that patients' symptoms of disswallowing or nasal regurgitation in the early postoperative period are mainly owing to phase III contractions occasionally encountered, because the pressure sometimes rise up to more than 100cmH2O and it may be higher than the degultition pressure or the obstructive pressure of nasopharyngeal cavity.
