    Recently, atelocollagen has been brought forward as an injectable material to the vocal folds instead of silicone. But in the clinical use, the results sometimes are not satisfactory because collagen volume decreases in the vocal folds after injection. A fundamental study of injected collagen in the tissues was recommended.
    3.0 and 3.5% collagen was injected in gluteus muscle of guinea pigs, and in the vocal folds of canines respectively, and observed under light microscope. The collagen injected in muscles is spread between muscle bundles but collagen injected in the lamina propria of the mucous membrane remained there making a mass. As a conclusion, a decrease of collagen volume after injection, is due to resorption of saline solution of collagen gel, spreading collagen between muscle bundles and a back-flowing of collagen from a hole made by an injection needle.

  • Analysis of Anastomic Leakage by Comparison of Manual and Stapled Anastomosis

    A disturbance in blood flow in the gastrointestinal anastomosis can be considered as one of the causes of incomplete suture which is one of the postanastomosis complications. In order to determine the relationship between blood flow and incomplete suture, manual and stapled anastomosis were performed in adult mongrel dogs, and then measurement of tissue blood flow by the hydrogen clearance method, pathological examinations of tissues and pressure test were performed. The amount of blood flow in the manual anastomosis at 7 days after surgery was increased to 79.6±15.3 ml/min/100 g in compared with the values obtained immediately after surgery, while blood flow in the stapled anastomosis was 52.3±15.6 ml/min/100 g. Incomplete suture occurred in 2 of 10 manual anastomosis and 1 of 10 stapled anastomosis, and the frequency of incomplete suture was lower in case of stapled anastomosis even though the blood flow was less than 40 ml/min/100 g. Esophageal reconstruction had been performed on 234 patients at our department, and incomplete suture occurred in 24 of 91 cases of manual anastomosis (about 26.4%) and in 15 of 143 cases of stapled anastomosis (about 10.5%) . The frequency of stump disruption was also lower with stapled anastomosis in clinical materials.

    Fifteen patients who survived for at least 5 years (long-term survivor group) and 24 patients who died of recurrence within 5 years after surgery (early death group) of the esophageal cancer in our department during the period 1964 and August 1984 were examined. We compared age, sex, location of lesion, X-ray type, longitudinal diameter of tumor, depth of invasion, lymph node metastasis, histological stage, blood vessel invasion, lymphatic invasion, therapeutic effect of preoperative radiation, and presence or absence of postoperative adjuvant therapy of two groups in order to study the factors affecting the postoperative prognosis of esophageal cancer. The results were as follows: 1) The incidence of lymph node metastasis in the long-term survivor group was lower (P=0. 003) than in the early death group and the number of patients in whom preoperative radiation was therapeutically effective was greater (P=0. 040) in the long-term survivor group than in the early death group. 2) The number of female patients was higher in the long-term survivor group, and the number of patients without blood vessel invasion, v (-), tended to be greater in the long-term survivor group. 3) There were 8 patients who survived for more than 10 years among the 15 long-term survivors, and longest survival period was 21 years. 4) Cumulative survival rate of 5 and 10 years were 37. 8% and 31. 0%, respectively, for 38 patients with curative resection.

    A case of large laryngeal hemangioma which almost occluded the laryngeal inlet is reported. A 39-year-old male visited our clinic with complaints of snoring and apnea while sleeping. Neither dyspnea nor hoarseness was noted on physical examination. The tumor had a wide pedicle attached to the left arytenoid and the left aryepiglottic fold, and was removed by lateral pharyngotomy. The excised tumor was balloon-shaped and had a diameter of 3 cm. Histological examination revealed that the tumor was cavernous hemangioma.

    Almost the entire malignant tumors arising in the larynx are the laryngeal carcinoma. Laryngeal carcinosarcoma, in which carcinoma (epithelial malignant tumor) coexists with sarcoma (interstitial malignant tumor) in the same organ, is rarely found.
    In this report, we described a recent case of carcinosarcoma arising in the epiglottis of a 74-year-old man who visited our hospital with a complaint of nocturnal dyspnea. We discussed the its clinical feature, pathological findings, and the treatment as well as a review of the literature of this disease.
    The carcinosarcoma in this case was extremely rare and it was comprised of squamous cell carcinoma, adenocarcinoma and sarcoma component which could be confirmed immunohistochemically.
    This carcinosarcoma was considered to be a true carcinosarcoma.

    A 64-year-old female with hemangioma was reported. Discomfort of the throat was the only presenting symptom. Indirect laryngoscopy showed a tumor on the right pyriform sinus. Microlaryngoscopic examination showed a pedunculated tumor originating from the right pyriform sinus. The tumor was dark red and elastic soft. It was completely excised by laryngomicrosurgery. The pertinent literature is reviewed.

    The cervical tumor observed in five cases extending to the upper mediastinum was extirpated in our department by using a combination of a neck dissection and a longitudinal median sternotomy. These tumors were originated from thyroid, thymus, and cervical nerve. The longitudinal median sternotomy is a useful approach to the upper mediastinum in order to keep a good operation view, to decrease bleeding volume, and to shorten operation time. Therefore, this approach should be adapted more for the extirpation of cervical tumor which extends to the upper mediastinum and for a mediastinal dissection.

    A 5-month-old male with many foreign bodies, such as a 500 yen coin, two 50 yen coins, eleven 1 yen coins and five iron balls was reported. When his mother brought him to our hospital at first time, one 500 yen coin was in his pharynx and was removed. After this episode, he was taken to his home. However, he was brought to our hospital, again on the same day because he vomitted two iron balls. The roentgenography revealed two iron balls and three coins in his esophagus. After removal of these foreign bodies, we confirmed no other foreign body and then he was admitted.
    During the hospitalization, his mother brought us five 1 yen coins and one iron ball and said he had vomitted these things. The roentgenography indicated five 1 yen coins in esophagus again and those were removed.
    This was a rare case of repititious, duplicate esophageal foreign bodies and we thought these were caused by an abnormal behavior of his mother which was given rise by her pregnancy.

    The complication rate after emergency tracheostomy is two to five times greater than that after elective procedures. One of the main causes of the high risk of complications in emergency tracheostomy appears to be due to the amount of time required to open the tracheal stoma.
    Therefore, simple and fast procedures are mandatory. We have developed a new procedure as follows: A horizontal skin incision is performed. Strap muscles are dissected and retracted laterally. A transverse cut between tracheal rings below the thyroid isthmus is performed up to 2-3 mm above the membranous portion of the trachea. Cut ends remain open naturally because of the elasticity of the trachea. Skin and tracheal cut-ends are then individually ligated by interrupted sutures.
    We have used this procedure during the past three years and have not experienced any major complications.
    This demonstrates the clear advantage and the more physiological nature of the procedure over various other incisions of the tracheal wall.
