  • Motility of the Ileum and its Relation to Diarrhea

    In our department, it has been already demonstrated that in dogs, subtotal colectomy with ileoproctostomy (Group 2) has brought about increase in ileal peristaltic movement in an early period within 1 to 4 weeks, and subsequent functional hypertrophy of ileum. In this paper is studied the influence of intraluminal pressure of the residual rectum and peristalsis of the ileum.
    Seventy-nine mongrel dogs were used and were operatively devided into 3 groups so that Group 1 consisted of those with subtotal colectomy with abdominal ileostomy; Group 3, Subtotal colectomy with ileoproctostomy and anal Sphincterotomy and Group 4, Subtotal colectomy with ileo-segmental colostomy with colostomy. Postoperatively were recorded the measurments of intraluminal pressure of rectum, the electromyographical change of terminal ileum, the peristaltic movement with X-ray cinematography and measurments of the thickness of muscular layer of terminal ileum, in addition to body weight, serum protein, hematocrit, cross findings of feces and mouth anus transit time.
    In these experiments all animals but one of Group 1 died within 2 or 3 weeks. Group 2, Group 3, Group 4 and Group 1 were thus observed in order of improvement of general findings. The mean intraluminal pressure of rectum was 164 mm H2O in Group 2 and 65 in Group 3 and 4. In Group 3 and 4, peristalsis of ileum was much decreased and the hypertrophy of ileal muscular layer was found not meaningful earlily in the postoperative period.

    As a result of using the injection with 5 percent phenol in an almond oil which is named PAO newly made by Torii pharmaceutical Company for 20 cases of Internal Hemorroids for the purpose of stopping their bleeding, the following efficacy is reported:
    Extremely good in 15 cases.
    Good in 5 cases.

    In Europe and America, 5 % Phenol-Almond Oil was used generally for the treatment of hemorrhoid and some clinical cases were reported of fair .results.
    In order to examine of hitological reaction in infused region this oil was injected in subcutanous tissue of colon in dogs and histological study was performed by lapse of times.

    PAOSCLE consists of 5 % phenol in almond oil, effect of which on hemorrhoids and other anal diseases was clinically studied. The effectiveness of the agent was judged on the basis of the calculating table devised by, Sakabe and others.
    The agent was remarkably effective for bleeding from hemorrhoids. The rate of effectiveness on the first trial was 97.0 %, 66.0 % and 66.7 % for the first, the second and the third grade hemorrhoids respectively. The rate of effectiveness on the second trial improved up to 100.0 %, 88.7 % and 93.3 % respectively. The data show bleeding from hemorrhoids is suspended by the agent in almost every case of hemorrhoids. The result was especially good when the amount of injection was 5 cc or more at one trial.
    The effect was also noticed as for the mild prolapse of hemorrhoids, although in that case repeated injection was required. The bleeding from prolapsed mucosa in the cases of so-called " Whitehead-anus " also favorably responded to the injection. However, no effect was noticed in rectal prolapse.
    Any influence on liver functions or hematologies were not observed. The side-effect was nil except only a case of temporary malaise. Sclerotherapy with such a low-density agent as PAOSCLE is quite free from such harmful results as induced by conventional necrotherapy.
    Sclerotherapy with PAOSCLE is recommended as a measure of choice before the trial of operation.
