    Recently, remarkable progress has been made on the remote results in the cases operated on colorectal carcinoma, and those participating factors are operation-technique, deeply reaching degree, metastasis to lymphatic nodes and remote metastasis. However, similar to the conventionaldiscussion on benign tumor, there is less report of statistical observation considering the defensive factors in view of the bio-antitumorous nature. We have, therefore, noticed the interstitial reaction at the border-face between the cancer-focus and the normal tissues, namely, the fibrillation and the appearance of the wandering cells, thus we reviewed the remote results of surgical cases on colorectal cartinoma.
    In our clinic, 35 caces were surveyed on the relationship between histological findings and prognosis in the patients treated with the operation for colorectal carcinoma in the period from 1959 to 1969, and we have reviewed the factors which seem to influence on the prognosis by means of our devised diagram-method. As a result, the appearance of the wandering cells was proved to be a very important factor of prognosis.
    As the kinds of the wandering cells, we have noticed the nature lymphocytes, neutrocytes, plasma-cells, eosinophilic cells, fibroblasts, monocytes (containing rather big cells with fine cells of round reticular cells, lymphoblasts, and histocytes), as well as mastcells, thus we have reviewed these cell'incidence and distribution.

  • Criticising on its Partial Fistulectomy

    Total and subtotal resection of. the anus as radical surgery for complicated anorectal fistula deep and wide spread all around the anus does not leave so serious complications as having been believed.
    Even if internal and external sphincter are excised completely around the anus, anal function is able to remain without so serious disturbance as long as the surgery is performed following the cardial principle of anorectal surgery being accompanied with proper post operative treatment and after care. But it is quite seldom to encounter such serious case, which needs total resection of the anus. I have only three cases of the total resection among 500 cases of fistula for last 5 years. These result may reveal that puborectal sling is most important factor to keep sphincter continence.
    I consider, it is quite difficult to expect its permanent and complete cure by partial fistulectomy resecting only primary opening and main tracts.

    There is no report of experimentally developed colon cancer in the dog up to date. In this paper, the experimental high grade atpyical glands of colon in dog which was expected to developed into adenocarcinoma was reported. Administration procedure of MNNG was as follows;
    The tip of silicon tube was fixed in the ascending colon through the vermiform appendix and the other end of tube was carried out to the back of the dog through the subcutaneous tunnel. Solution of MNNG with 3 g/1 tween 60 were injected through this tube twice a week in 8 dogs.
    The endoscopic examination were performed regularly in each dog after the start of administration of MNNG. The administration of MNNG were varied depending on the general condition of the dogs.
    In one dog which was underwent of administration of 2mgMNNG/kg for weeks and 1 mg/kg for 41 weeks, the endoscopy demonstrated the lesion similar to IIc+IIa, type of early cancer. Biopsy of the lesion revealed the atypical glands in deep mucosal layer.
    By autopsy, this lesion was confirmed as high grade atypical glands and protrusion of its glands into submucosal layer was proved histologically.

    We intented the evacuattion of the bladder by electrical stimulation for the vesical dysfunction, which was one of the major complications of rectal cancer resection. We stimulated the bladder from the lumen by monopole electrode attached to a catheter.
    In 38 normal dogs and 6 paraplegic dogs, the highest intravesical pressuse was recorded at the points near the ureterovesical junction. The best divect intravesical stimulation was obtained by a frequency of 60-100 cycle per second, a duration of 2-5 millisecond monophasic square wave, 10-20 voltage, 5-7 second intervals, though there was a tendency to more voltage needed in paraplegic than normal dogs.

    Selective arteriographic findings were analysed in 17 cases of colonic cancer and 3 cases of ulcerative colitis comparing with microangiography and histological finding of resected specimen. All of 17 cases revealed advanced cancer and ulcerated type. Histological diagnosis were well differentiated adenocarcinoma. The preoperative arteriographic signs generally showed the changes in protuberant areas in microangiographic study and consequently indicated the extend of tumors. The microangiographic pattern in the tumors were very chaotic. In areas of ulceration hypovascular and fine tumor vessels, in protuberant areas hypervascular and much tumor vessels, and in outside of tumor normal vascularity and small changes of the course in part were demonstrated.
    Arteriography of ulcerative colitis was performed during clinical and histological early remission stage in all cases. The microangiographic pattern varied in parallel with histological features. In areas of ulceration, hypervascularity in submucosal vessels and irregularity in part in penetrating vessels were demonstrated. In area of regenerative epithelium, mucosal vessels were similar to normal, but dilated and hypervascular submucosal vessels were demonstrated.
    Capillary blush in the preoperative arteriographic signs generally showed dilated vessels in submucosa and consequently early venous drainage is suspected to be depended on A-V shunt in submucosa.

    A clinical and pathological study was performed in the group of 10 cases of Crohn's disease and as a control, 33 cases of other inflammatory intestinal diseases. The general status of patients of Crohn's disease revealed more favorable both in pre- and postoperative period than that of patients having ulcerative colitis or intestinal Behcet.
    No single surgical death was observed in the former groups, however, postoperative recurrence developed in 4 cases. Moreover, the many patients persistently had abdominal complaints even in the late portoperative period.
    Histopathologically, recurrence was noted exclusively in the small intestine where the lesion was extensive and associated with marked edema and congestion, in large intestinal Crohn's disease, there found much more fibrous and circumscribed tumor formation.
    It was of special interest that in the recurrent cases the specimen taken from intestinal stump at the first surgery showed more marked edema and massive eosinophilic infiltration than in the non-recurrent cases.
    These findings were concluded as useful to determine the surgical indication prior to surgery or extent of bowel resection at surgery so as to prevent recurrence of the disease.
