    Quantitative anorectal manometry by open tipped method modified cylinder core was examined on 70 normal subjects ranging birth to adult for the evaluation of physiological anorectal function and also discussed the methodology of pressure studies.
    At the neonatal period, especially during first week of life, anorectal pressure is very low, as well as recto-sphincteric reflex shows inactive. However, greatest rate of growth takes place unti 2 months after birth. The length of anal canal develops markedly in the second decade. From the physiological point of view, we divided functional anal canal into 3 parts, ie, puborectalis zone, internal sphincter zone, external sphincter zone. With introduction of increasing volume of air into the rectal balloon, the internal sphincter zone showed gradual decrease of basal tone and the puborectalis zone a similar reduction of tonic activity.
    For the assesment of anorectal defecative motility, we advocate to measure the " REFLEX ANORECTAL PRESSURE PROFILE " which composed of projecting the pressure fall on the resting anorectal pressure profile. It demonstrates that the anorectum reduces its tone from upper part to downward with proportional to the increase of rectal pressure.

    279 patients with a rectal cancer and 120 patients with a colonic cancer proven histologically on operative specimens were seen at the Aichi Cancer Center Hospital between Dec. 1964 and Dec. 1972. To promote an earlier diagnosis, the presenting symptoms and signs in relation to the Dukes' classification and the site of the cancer were studied.
    399 patients were classified into 103 patients of Dukes'A, 90 of Dukes'B and 206 of Dukes'C. Rectal bleeding was much more common in patients with cancer of the rectum and the sigmoid colon in the Dukes'A. Altered bowel habit and abdominal mass noticed by patients were relatively late symptoms. Although patients with a right-sided lesion and cancer of the transverse colon were more likely to present with abdominal pain in the Dukes'A, bleeding from the large bowel, i.e. not only over bleeding but also positive occult blood (more than + + by Benzidine method) and/or anemia (less than 350×104mm3. in R.B.C. and⁄or 70% Sahli in Hb) was confirmed in the vast majority of patients.
    It is, therefore, concluded that testing occult blood in the stools plays an important role in early detection of cancer of the large bowel.

    Two anesthetic methods in minor anorectal surgery was compared in incidence of complications in 92 patients in the period of 2 years and 9 months from 1974 to September, 1976. Transsacal block was used in 37 patients and spinal anesthesia in 55 patients.
    In transsacral block, incidence of low blood pressure was 2.8% significantly less than 27.3%, catheterization for postoperative urinary retention 27.8% less than 38.2%, postoperative vomiting 0% less than 5.5%, postoperative headache 2.8% significantly less than 16.4%, and infection of urinary tract 0% less than 3.6% in spinal anesthesia.
    Under consideration of above results, transsacral block is not so difficult in technic and had less incidence of complications than spinal anesthesia.

    In recent two years 43 cases of diverticular disease have been found at our cinic, of 740 cases examined with barium enema. 3 cases of 43 cases were operated, one was at the right side and other two were at sigmoid colon.
    A case of diverticulitis coli with stenosis of sigmoid colon is reported in this paper. The patient was a 57-year-old man with a complaint of disturbance of defecation. He was already diagnosed of diverticulosis coli 2 year before, and had been used to be well with drug treatment. He noticed again the same symptom at this time, and consulted our clinic. On the films of barium enema, severe stenosis of the sigmoid colon was found, and after this examination he had suffered from high fever for a week. On the endoscopical examination, neither tumor nor ulceration revealed, and the mucosa relatively smooth, but the lumen of the colon was severely narrowed. So that diagnosis of signosis of sigmoid cancer was negated from the endoscopic findings. Radiographically, diverticula were seen on the whole tract of the colon, especially at the ceacal and sigmoid portion.
    From the clinical course and symptom, surgical treatment was indicated, so subtotal colectomy was performed. A tumor of the size of the fist formed at sigma of the resected specimen, of which surface of the section, increased muscular layer and the development of fatty tissue with microabscess formation were prominent.

    The ulcerative colitis is characterized in a chronic inflammation and ulcerative change involving the diffuse region of the colon and rectum. It's typical type is easily diagnosed by x-ray and endoscopy, but it is still difficult to differentiate its atypical type from other nonspecific inflammatory colonic disease.
    The first case is a 54-year-old male and he visited us complaining of massive hematochesia. In the endoscopic finding, mucosal hypermia, edema and bleeding were remarkable but pseudopolyposis was not clear. In this case, conservative treatment including steroid administration was not effective and surgical treatment was performed. On examination of the excised specimen, it did not show a typical change of ulcerative colitis.
    The second case is a 45-year-old male. The patient complained of bloody stool and severe diarrhea over 20 times a day. In this course, anorexia and melena became remarkable with diarrhea. The most cparacteristic findings of the endoscope inserted 10 cm from the anal verge was hemorrhagic-mucopuriilent discharge and hyperemic swelling of the mucosal membrane.
    The diagnosis of fulminant ulcerative colitis depends upon the clinical features, abnormal biochemial findings, and the romonscopic findings was performed, Intausive conservatic mauagent was not successful in this patint and dead in the sixth day. Gallagher described the management of patients with fulminant ulcerative colitis, of which 22 cases undewent early surgery with 9% mortality rate.
    Prolonged conservative therapy or delayed surgery were resulted in 61% mortality rate. In an attemp to lessen mortality, colectomy should be recommended at an early stage of the illness if no prompt improvement occured with conservative therapy.
