    Inferior mesenteric angiography has been performed before and after intra-arterial injection of noradrenaline (NA) in 15 patients with carcinoma of the colon and rectum.
    We discussed the usefulness of pharmacoangiography to evaluate the size of mass and tumor growth through the bowel wall.
    To compare the vascularization of tumor, angiography of resected specimen was also performed. Following NA injection, constriction of inferior mesenteric artery was observed in the majority of our cases.
    In 5 cases given NA 10 μg or more, marked arterial constriction caused failure of the filling of the tumor and prevented the correct diagnosis. Diagnostic effect of NA 2-5 beg injected in other 7 cases was approximately the same before and after the administration.
    In other 3 cases 2 to 5 μg of NA improved the diagnostic value to define the delineation and infiltration of the tumor.

    Eight hundred and twenty seven cases with rectal prolapse have been reported from 87 hospitals in Japan and a statistic survey was carried out on these cases.
    The rectal prolapse in our country was more common in the male than in the female, and the ratio of the male to the female was five third. As the age distribution concerned, the male patients were rather young than the female in average.
    Types of the rectal prolapse were classified as to be followed ; complete rectal prolapse (67%), incomplete prolapse (26%) and others.
    The duration of symptoms was 14.9 years in average, and 35% of the all cases were suffered from the rectal prolapse for more than ten years.
    Fourteen percent of the patients were treated conservatively and the rest of the cases were to be cured surgically. Fifty seven per cent of the surgical cases were done through the anus and 36% were treated through the abdominal approach.
    Various kinds of the surgical correction were performed on these patients, and the most popular method was Gant's multiple ligation of the protruded rectal wall. So called sling operation as described by Ripstein and others has not been popular in Japan, but the cases treated in. this procedure are increasing recently.

  • Double Blind Group Comparative Trial

    On this report, the effect of Sc 295 on internal hemorrhoids was studied with the inactive placebo by the double blind group comparative trial.
    The subjects were 144 cases administrated with Sc 295 and 141 cases administrated with the placebo, totaling 285 cases.
    1) The global utility rating showed the effect of Sc 295 more significant than that of the placebo in one week and 2 weeks.
    2) The effects of Sc 295 on pain, bleeding and swelling were especially significant.
    3) The incidence of the side effect of Sc 295 were 9.7% when compared with that of 5.5% of placebo. However, there was no significance between them.
    4) Laboratory examinations during this trial were investigated on 50 cases and almost all of them were in normal limits.

    Tuberculosis of the anorectal region is now a rare disease in Japan. However, the possibility of anorectal tuberculous ulceration must be considered when any patient have chronic perianal lesion which resists usual treatments.
    A 65-year-old man was admitted to our clinic with the complaints of pain and hemorrhage of perianal ulceration. The onset was perianal eczema-like lesion which occurred about two years prior to the admission.
    Chest X-ray examination revealed active pulmonary tuberculosis with positive acid-fast bacilli in the sputum, and histological examination of perianal lesion showed typical tuberculous granulomas with epitheloid cells and Langghans giant cells, surrounding early stage of necrotic foci, and diagnosis of secondary tuberculosis of anorectal region which was classified as ulcerative tuberculosis by Bacon was made.
    The patient was treated with SM, PAS and INAH. The perianal ulceration disap-peared completely after six months.
