  • A Case Report and Review of 60 Cases in Japan

    A 65-year-old female was admitted to Yamanashi Prefectural Central Hospital with the chief complaints of anal pain and anal bleeding. Digital examination of the rectum revealed a elastic hard tumor on the posterior wall just above the dentate line. A combined abdominoperineal resection of the rectum was carried out. Histological findings showed malignant melanoma of the anorectal junction. She died of local recurrence 14 months after the surgery. This is the 60 th case in Japan in the extent of our knowledge.
    Review of 60 cases in Japan is as following : Most anorectal malignant melanomas were situated at or near dentate line (histologically anorectal junction) and the posterior wall was chiefly affected (41.7 %). In 60.4 %, the estimated diameter of the lesion was 2 to 5 cm. Fourty two percent of the patients were between the ages of 60 and 69 years. The youngest patient was 25 and the eldest was 80 years. Eighteen patients were men and 42 were women a ratio of 3 :7. Of many symptoms, bleeding, constipation and pain were most frequently present. Five-year survival rate was 10.7 % and the average life expectancy after the diagnosis was 17.5 months.

    Among 851 resected cases of colo-rectal carcinoma, there were 54 cases with deliberate resections and 29 cases with accidental injuries of. the urinary tract. We studied those 83 cases in respect of reconstructive procedures of the urinary. tract. In 54 cases postoperative coures were almost uneventful. Among 29 cases, thirteen cases occurred accidental injuries of the urinary tract during surgery, however sixteen occurred postoperatively. Three cases with ureteric injury occurred above pelvic brim, were treated by end to end anastomosis of injured ureter.
    Postoperative course-of these cases were uneventful. In cases with-ureteric injuries occurred below the pelvic brim, uretero-neocystestomy should be performed for reconstruction of the injured ureter. There were eight cases with vesicoperineal fistula among 320 cases of abdominoperineal exsion of the rectum. Among those eight cases four have irradiated bladder. In such a case with Irradiated bladder, urinary diversion should be performed.

    A 62-year-old female had a one-year history of a slowly enlarging eczematous eruptions on the perianus, at which time she had treated for eczema with ointment but without improvement. Physical examination showed an irregularly shaped eczematous lesion at anterior anal margin, measuring 4.5 X 2.5 cm in dimension. This was accompanied with induration of walnut size. Biopsy revealed epidermoid carcinoma.
    A combined abdomino-perineal resection of the rectum was carried out with additional wide resection of the posterior wall of the vagina. Histological findings showed squamous cell carcinoma in situ. There was no invasion as far as examined in multiple sections and these did not involve the rectal mucosa beyond the dentate line.
    A case of squamous cell carcinoma in situ of the anus. was presented.

    A 29-year-old housewife was admitted with compleints of pain and a painful mass in the left lower quardrant of abdomen.
    A double contrast barium enema study showed marked narrowing of the distal segment of the discending colon with irregular margin and gradual transition to adjacent normal bowel. Nodularity of the mucosa without ulceration was shown. A inflammatory lesion was suggested, although roentgenological findings simulated linitis plastica carcinoma.
    Macroscopically the excised specimen showed marked thickening and rigidity of the wall. Histopathologically, in two of 31 sections in whole specimen surfur granules were seen, which actinomycosis was diagnosed.

    Prednisolone suppositories were administered in treating 192 cases of ulcerative colitis and8 cases of radiation colitis. In both these types of colitis, main active lesion were in the rectum and sigmoid colon.
    The following results were obtained :
    1. Ulcerative colitis
    Mucous blood in the feces and bloody diarrhea had disappeared in 90 cases (47 %) after two weeks, and in 134 cases (70 %) after four weeks of administering the suppositories, by which time improvement of symptoms had developed in 176 cases (92 %). Deterioration was observed in only one case (0.5 %).
    On the colonofiberscopic examination in these cases, however, complete remission could be?found in only 57 cases (36 %), and improvement of mucosal changes in only 89 cases (56 %) even after eight weeks. Deterioration was not observed.
    2. Radiation colitis
    Mucous blood on the feces and bloody diarrhea had disappeared in 5 of the 8 cases after two weeks of administering suppositories, but colonofiberscopy revealed that a decrease in inflammation, erosion and bleeding had occurred in only one case after the same period of time.
    3. There was no adverse reaction except in two cases, in which partial stimulation for rectum occurred,
    To sum up, this suppository is extremely effective in treatment of ulcerative colitis. While it is being administered, however, regular or periodic endoscopic examinations should be made to confirm whether dosage might be decreased or suspended altogether, because of the long period of time between the disappearance of bloody feces and the achievement of endoscopic remission.
