    The authors reviewed 281 cases with overt lower intestinal bleeding seen at the Aichi Cancer Center Hospital during the period from January 1977 to April 1979.
    Among the patients with overt rectal bleeding, advanced colorectal cancer was the most common, accounting for 30.9%. Polyp (19.2%), hemorrhoids (17.1%), proctocolitis of known etiology (drug-induced, radiation, infections, etc.) (13.1%), chronic ulcerative colitis (10.6%), diverticulosis (2.8%) and early colorectal cancer (2.5%) were seen in this order of frequency. Furthermore, a number of patients were found to be positive on fecal occult blood testing.
    Rectal digital examination and fecal occult blood testing are easy to perform with inexpensive cost and the obtained fingings can provide important clues to the diagnosis of various colorectal diseases.
    In order to prevent a symptomatic patient from delay in visiting a doctor, education via mass media will be required for increasing intestinal diseases.

    To evaluate the defaecation function after surgery, anorectal manometric studies with balloon method were performed on 89 patients with anorectal diseases and 70 patients who had undergone various kinds of sphincter-saving colorectal surgeries, compared with 46 healthy adults as control.
    The results obtained are as follows.
    1) "Slow wave", one type of anal pressure waves at the resting state, is supposed to be caused by denervation of the sympathetic nervous system.
    2) Postoperative disturbances of defaecations are supposed to be caused not only by lowered anal sphincteric functions (anal pressure, rectoanal reflex) but also by lowered reservoir functions (rectal compliance, maximal tolerable volume, continuous relaxation volume).
    3) The formula, f (the discriminant function)=0.035x+y-5.75, deviced by the author can clearly discriminate between the group of "good" or "fair" and that of "poor" following clinical assessment. Two variables x and y indicate resting anal pressure and rectal compliance, respectively, measured in the manometric studies. Hence, the author is confident that the cases of "f>0" can be judged as good or fair and those of "f <0" as poor.

    The radical hemorrhoidectomy was performed in 56 cases of hemorrhoids with a high power carbon dioxide laser knife. The operation was performed according to the Milligan-Morgan method and/or to wedge-shaped mucosal resection with a laser knife following ligatures of hemorrhoidal arteries. Defocused beam of 20W was mainly used as irradiating laser. In the hornorrhoidal operation with the laser knife, resection can be proceeded smoothly by controlling bleeding. Virtually few hemostatic ligation was necessary during the operation, and pressure application for hemostasis to postoperative wound, was seldom required.
    The advantages of the laser hemorrhoidal operation are as follows. 1) Reduction in bleeding during operation. 2) Good operative view. 3) Simplification of the operative method. 4) Less postoperative edema and pain. 5) Shortening of hospitalization.
    Although there are many indication of laser in anal diseases, much can be expected by the use of laser in the hemorrhoidal operation in the future.

  • Evaluation of Palliative Treatment

    Minute changes of the surface of hemorrhoidal mucosa could be clearly observed with an endoscope. 50 patients with internal hemorrhoid were treated with Tribenoside (1200 mg/day) and Steroid- suppository for 14 days. Before and after the treatment hemorrhoid was inspected with a rectoscope (SL-A type). After the treatment the following macro-scopic findings were observed.
    1) Disappearance of superficial inflammation and increase of capillary translucency on the hemorrhoidal mucosa.
    2) Disappearance of hemorrhagic erosion.
    3) Decrease of dilated dark-red vessels on the hemorrhoidal mucosa.
    4) Disappearance of hemorrhoidal varices.
    Endoscopic inspection of internal hemorrhoid is useful for evaluation of palliative treatment of the disease.

    The findings of 3239 barium enema examinations performed at Seoul National University Hospital (Jan. 1, 1980-Oct. 30, 1981) were reviewed.
    9 of 3239 cases were found to have colonic diverticular diseases. The ages of the patients with this disease were ranged from 35 to 66 (mean 56). Sex difference showed the predominance in the males (6) compared with the females (3). On the distribution of diverticula, 8 cases were located in the right side of the colon and one case only in the left side. These findings were consistent with many reports in Japan. In additions, the complications of the diverticular disease (i. e. diverticulitis, perforation, bleeding, and so on) were not found at all.
    On the contrary to Caucasian and other people, colonic diverticula in Korea as well as Japan are often found in the right side of the colon. It is yet obscure whether the diverticular disease of the right side is characteristic to Mongolian people. It is not also evident whether dietary factors are concerned in the right sided diverticula, as already pointed out in the left sided ones. These problems must be dissolved by further studing of the diverticular disease of the colon in other Mongolian countries.

    The patient was a 79-year-old female, who was admitted with abdominal pain on Descember 2, 1980. Barium enema rivealed filling defect of the transverse colon. Curative right hemicolectomy was performed on December 16, 1980. Pathological findings showed malignant lymphoma, non-Hodgkin, hystiocytic type. Postoperative course was eventful. Since such a case is not frequently seen clinically, so Japanese reference was riviewed and discussed with Japanese five cases reported in detail.
