    Urodynamic studies were performed pre- and postoperatively on 33 patients who underwent radical operations for the rectal cancers. Information on urinary dysfunction was investigated by a questionnaire at the same time. Urodynamic studies consisted of cystometry and uroflowmetry. A questionnaire concerning urinary dysfunction consisted of urinary frequency, urge to urinate, urinary incontinence, sense of residual urine and dysuria. Among the results of urodynamic studies, maximum desire to void, residual urinary volume and compliance of the urinary bladder have a close relationship with clinical symptoms. Urodynamic evaluation of before and after surgery has a strong relationship with clinical symptoms, and improvement of data implies the recovering state of urinary dysfunction.

    The role of bcl-2 and p53 gene in development of colorectal cancer is not fully known. The purpose of this study was to clarify the relaion of bcl-2 and p53 gene in the Carcinogenesis of the colorectum with special reference to the apoptosis.
    Thirty-eight adenomas and twenty cancers in adenoma (CIA) of the colorectum were examined pathologically and immunohistochemically. CIA was divided into adenoma and carcinoma areas. Expression of bcl-2 and p53 was detected by LSAB. The apoptotic index (AI) was evaluated histologically using the TUNEL method.
    Positive rates of bcl-2 overexpression of the carcinoma area, the adenoma area and the adenoma were 10.0, 45.0 and 37.9 % respectively. Positive rates of bcl-2 overexpression of each atypical grade of the adenomas were severe : 66.7, moderate : 35.3 and mild : 14.2 %. Positive rates of p53 overexpression of the carcinoma area, the adenoma area and the adenoma were 75.0, 20.0 and 16.2 % respectively. AIs of the carcinoma area, the adenoma area and the adenoma were 2.17±0.88, 5.07±2.75 and 5.89±2.37 respectively.
    An inverse correlation was found between bcl-2 and p53 overexpression in the carcinoma area, suggesting that mutated p53 may regulate apoptosis in the carcinogenesis of colorectal adenoma.

    This study was conducted to evaluate the symptomatic relief and functional consequences of sliding skin graft in patients with stenotic anal fissure. Thirty-four patients with stenotic anal fissure were retrospectively compared before and one month after fissurectomy with sliding skin graft for symptomatic relief and manometric results. Maximum resting anal pressure, maximum squeeze anal pressure, and high pressure zone were recorded. The results were compared with 83 controls free of anorectal disease and were analyzed using the t-test. Preoperative resting anal pressure fell significantly compared to controls, and postoperative resting anal pressure was significantly lower compared to the preoperative one.
    All patients except three had symptomatic relief after one month and no recurrence was noted after one year.
    Sliding skin graft was a good procedure for anal fissure with stenosis in evading recurrence, complicative incontinence and symptomatic relief, however, was thought to be invasive.

    Hepatectomy and intraarterial chemotherapy after hepatectomy for liver metastasis from colorectal cancer have been performed in our depatment. Intraarterial infusion chemotherapy has been performed for unresectable liver metastasis, too. One hundred three cases of liver metastasis from colorectal cancer were studied. The cases were divided into groups according to radicability of original colorectal cancer, with or without hepatectomy and with or without Intraarterial chemotherapy. Group 1 is cur C of origin. Group 2 is cur A or B without hepatectomy. Group 3 is cur A or B with hepatectomy. Each group was divided into group without Intraarterial chemotherapy (a) and group with one (b). 1-a has 20 cases, 1-b 7 cases, 2-a 16 cases, 2-b 15 cases 3-a 28 cases, and 3-b 17 cases. The survival rate of group 3 was better than that of group 2. The survival rate of group 2 was better than that of group 1. There was no significant difference of survival rates between 1-a and 1-b. The survival rate of group 2-b was significantly better than that of group 2-a. The survival rate of group 3-b was significantly better than that of group 3-a. Hepatectomy and intraaterial chemotherapy after hepatectomy for liver metastasis from colorectal cancer were effective.

    Steroid induced avascular necrosis of femoral head (ANF) in three patients with ulcerative colitis (UC) was reported. The ages of the patients (two females and one male) were 24, 30, and 55 years. Two had total colitis and one had left-sided colitis. They were treated with prednisolone for recurrent active disease. The duration from onset of UC to onset of ANF were 15, 31, and 48 months. After the onset of ANF, two received steroids continuously for active disease. Total proctocolectomy with ileal pouch-anal canal anastomosis was carried out for their colonic disease in our hospital. Cumulative prednisolone doses were 9.8 and 11 grams, when they underwent colectomy. Postoperatively, one needed total hip replacement for her collapsed right femoral head. In the third patient, steroids were tapered after ANF was diagnosed. The disease remained in remission there after, and her pain was gradually relieved by protection of weight bearing. Cumulative prednisolone dose was 30 grams.
    UC patients with long-term or high dose steroid therapy have high risk of ANF, which must be recognized and treated at an early stage.

    The patient was an 18-year-old male with a past history of mental retardation since 5-years-old. Follow-up was done in our department, and the diagnosis was Peutz-Jeghers syndrome in May 1992. Blood examination revealed anemia in February 1995. On August 14, 1995, polypectomy of the gastrointestinal tract was carried out because bleeding from polyps of the small intestine was suspected. Laparotomy and intraoperative total enteroscopy from the esophagus to the rectum was carried out under general anesthesia. Most of the polyps over 5mm in size were resected by endoscopic and surgical polypectomy. It could be concluded that intraoperative endoscopic and surgical polypectomy for clearance of small intestinal polyposis is a choice of treatment in cases like this one.

    A 36-year-old female who had advanced descending colon carcinoma with extended infiltration to retroperitoneal space was successfully treated by surgical resections following an effective radiation therapy and sequential methotrexate-5-fluorouracil (MTX/5-FU) therapy for repeated local recurrences. Neither side effects nor complications were observed, and the disease free period has continued for three years and 6 months since the last (3rd) resection. A long-term survival case following radio-chemotherapy and surgical resections for each repeated local recurrence of colon cancer was reported.

    A case of GIST (gastrointestinal stromal tumor), arising in the rectum was reported, “GIST” has been applied to mesenchymal tumors that represent neither typical features of leiomyomas, leiomyosarcomas nor Schwannomas. Ultrasonography, colonoscopy, CT and MRI revealed a submucosal tumor in the lower rectum and aspiration cytology revealed that the tumor may have derived from smooth muscle, leiomyosarcoma. The tumor measured approximately 7 cm in diameter and was removed surgically. But immunohistochemical study showed that the tumor cells were negative for SMA and desmin, and focally positive for S-100 protein and NSE, and strongly positive for CD-34 and c-kit. An ultrastructural study failed to show any differentiation toward either cell type, which made the final diagnosis of rectal stromal tumor with focal neural differentiation.
    In literatur, 71 rectal tumors of 941 gastrointestinal myogenic and neurogenic tumors between 1986 and 1996 were reported in Japan. Of these 71, immuohistochemical study was done only in 9 cases, out of which diagnosis of 7 cases were controversial in comparison with the definition of the GIST. Thus it was thought that many cases with GIST were included in the 941 reported cases.
    Immunohistochemical and ultrastructural studies are necessary for the differential diagnosis of GIST, and gastrointestinal surgeons and pathologists must recognize GIST.

    It is said that one of the conservative therapies for hemorrhoids is to improve the blood flow of anodermal mucosal layers. In this study, the change of the anodermal blood flow was examined using a warm water irrigation toilet seat device. The blood flow of the anodermal mucosal layers was measured by the lazer doppler method, which compared the blood flow of samples between pre-irrigation and post-irrigation periods. The results were as follows : when the temperature of the irrigation water was set to 25 degrees, the blood flow during the post-irrigation period was almost the same as the pre-irrigation period. In all 6 cases, the post-irrigation blood flow increased compared to the pre-irrigation blood flow when the temperature was set to 40 degrees. In conclusion, it is considered that the warm water irrigation (40 degrees) by the toilet seat device is useful for improving hemorrhoids or fissures. In addition, this device is beneficial for early healing of the anodermal area after hemorrhoidectomy.
