    Abstract : The three-dimensional (3D) image data from cone-beam CT gives useful information for diagnosis and treatment in dentistry. 3D image evaluation permits the clinician to select a target region and analyze desired cross-sections at different angles and positions as required. Furthermore, teeth, jaws and surrounding areas can be evaluated in detail. In addition, the width and shape of hard tissues can be measured accurately. Conventional CT for medical use offers a slice resolution of 1 mm in contrast with dental CT resolution with a slice resolution from 0.1 to 0.4 mm. In other words, we can obtain approximately 10 times higher resolution and high-quality images with 3D dental CTs. Furthermore, the clinician’s expertise does not affect the quality of the obtained CT images, and reconstructed images can be obtained. In general, the files used for endodontic treatment, which range from 0.5 to 1.0 mm, can be accurately analyzed by dental CT. Furthermore, periapical lesions and the floor of the sinus and neighboring areas of teeth can be thoroughly evaluated and diagnosed. In our clinical evaluation, the extension of periapical lesions and the origin of root canals can be easily differentiated. The anatomy of single and multiple root canals on which root canal obturation has been performed can be fully understood. For these reasons, dental CT is essential for accurate diagnosis, prediction and prognosis in endodontic treatment.

    Abstract : Dentinogenesis imperfecta is an autosomal dominant inheritable disorder affecting an estimated 1 in 6,000 to 8,000 people. It is usually found as opalescent blue-gray or yellow-brown dentin of deciduous teeth. The enamel is prone to exfoliation or fracture and the dentin promptly wears, resulting in pulpitis or apical periodontitis. As pulp obliteration is typically too severe for conventional root canal treatment, extraction is often indicated. This case report describes endodontic treatments to preserve two non-vital permanent teeth of a patient with dentinogenesis imperfecta using a microscope and cone beam computed tomography.

    Abstract : The aims of this study were to investigate the effects of ofloxacin (OFLX), a new quinolone antibacterial agent, on rat periapical lesions, and to examine the behavior of cells expressing transforming growth factor (TGF)-β1 during the healing process of periapical lesions. After applying OFLX to experimentally induced periapical lesions, a significant increase followed by a drastic decrease in cells expressing TGF-β1 and considerable bone formation was seen around the root apex. Additionally, a part of the cells expressing TGF-β1 seemed to be macrophages. On the other hand, topical application of physiological saline resulted in only few cells expressing TGF-β1. However, lost bone tissues within the vicinity of the root apex did not recover well despite there being numerous osteoblasts on the bone surfaces. In addition, expression of IL-1α was considerably decreased in ofloxacin-treated rats, whereas numerous IL-1α-expressing macrophages remained in the lesions of physiological saline-treated rats. These results suggest that the time and extent of TGF-β1 expression may influence the course of healing, and that a transient increase of cells expressing TGF-β1 may play an important role in the activation of new bone formation during the healing process of apical periodontitis. The present study suggests that OFLX may shorten the healing process of periapical lesions, and may be therapeutically useful for acute alveolar abscesses to which it is difficult to apply irritant intracanal medicaments.

    Abstract : It is known that bacterial infection spreads not only in root canals but also around periapical tissues. Periapical lesions are an infectious disease, and so it is thought that identification of bacteria in periapical lesions and systemic administration and local application of antibiotics based on a susceptibility test are very effective. This case report describes the treatment of chronic apical periodontitis with a large periapical lesion by this method.

     The patient was a 40-year-old woman. An X-ray photograph of the problem tooth, a maxillary left second molar, showed a large periapical lesion. The exudates from the periapical lesion were used for the bacterial identification and antibiotics susceptibility test. Pseudomonas aeruginosa was detected in the exudates. According to the results, cefaclom was locally applied into the root canals, and levofloxacin hydrate was systemically administered. All clinical symptoms and anaerobic cultivable bacteria disappeared and then the root canals were filled. The prognosis was good 17 months after the root canal filling.

     This case suggests the usefulness of root canal treatment with identification of bacteria and systemic administration of accurate and adequate antibiotics for large periapical lesions, which are generally surgically treated.

    Abstract : This paper reports a good outcome obtained during 14 years of long-term follow-up after a nonsurgical endodontic procedure was carried out for a periapical lesion of a tooth. The patient underwent eco-mechanical root canal therapy as follows :

      ・Mechanically remove infecting factors from inside the root canal.

      ・Minimize the use of drugs while stimulating the body’s healing power to maximize homeostasis.

      ・Ensure a tight obturation at the root canal with no dead space remaining.

     This report indicates that eco-mechanical root canal therapy may bring a good outcome from the treatment of periapical lesions of teeth.
