ジャーナル フリーClinical application of Gray-scale ultrasound for the diagnosis of neck swellings with the exception of thyroid and salivary gland lesions is discussed. Thirty-two cases of neck swellings were examined, which include: 7 cases of inflammatory diseases, a castleman lymphoma, 3 cervical cysts, a ectopic thymic cyst, 2 epidermoid cysts, 2 hemangiomas, 2 lipomas, 2 neurogenic tumors, 5 malignant cervical tumors, and 7 cases of metastatic lymph nodes, in which invasion to the carotid artery were strongly suspected. We conclude: 1) Gray-scale ultrasound is successfull in diagnosing the swelling in any case of neck lesion. 2) Malignant tumors may exhibit the characteristic ultrasound patterns of benign tumor and inflammatory lesion. 3) As the carotid artery is a easily recognizable landmark with ultrasound, vascular invasion of metastatic tumors, more than 7mm in diameter, can be easily identified by examining the location and the shape of the carotid artery.抄録全体を表示PDF形式でダウンロード (13140K) -

ジャーナル フリーAs one of the iatrogenic diseases, a very rare case of incudostapedial joint dislocation by bouginage of Eustachian tube is reported.
A 48 year-old male was treated for otitis media with effusion with polizerization and bouginage of the Eustachian tube. During the bouginage the patient felt severe pain in the ear. Bleeding in the external ear canal followed by a severe hearing loss was noted. Eight months later the patient visited our clinic and audiological examinations revealed a conductive hearing loss of 50 dB with Ad type tympanogram.
Exprolatory tympanotomy revealed a luxation of the incudostapedial joint. The dislocated long crus of the incus was repositioned but a small space remained between the head of stapes and the lenticular process. Consequently, an autogenous bone fragment, trimmed in a suitable from, was inserted in this space.
To establish the possibility of damaging the joint by bouginage, a bougie was inserted through the Eustachian tube of a temporal bone specimen. As a result it was recognized that the bougie reached to the joint directly if it was inserted too deep into the middle ear cavity.抄録全体を表示PDF形式でダウンロード (5269K) -

ジャーナル フリーA case of psychogenic deafness was reported. A 12 year old boy who had suffered from sudden deafness of the right ear since two years suddenly complained of severe deafness in the left ear. In spite of subjective hearing loss, speech audiometry and auditory brain stem response revealed: normal hearing. Through psychotherapy, hearing loss became normal 3 months after the onset.

ジャーナル フリーDeglutition is the transfer of solids and liquids from the oral cavity to the stomach brought by complex coordinated reflex action. This action is divided into three stages, the oral, the pharyngeal and the esophageal stage. Once the swallow has begun, all the essential muscles participate in a stereotyped way, especially in the pharyngeal stage.
This paper deals with the effect of the anesthesia and the kind of bolus in the pharyngeal stage of swallowing in dogs, studied with electromyography.
In these series of investigations, operative exposure was followed by implanting a 0.12mm surgical stainless steel wire electrodes for electromyographic testing into the three elevators of the larynx, geniohyoid muscle mylohyoid muscle and thyrohyoid muscle, the four pharyngeal constrictors, palatopharyngeal muscle, hyopharyngeal muscle, thyropharyngeal muscle and cricopharyngeal muscle, one puller back of the tongue base styloglossal muscle and the three intrinsic laryngeal muscles, thyroarytenoid muscle, posterior cricoarytenoid muscle and cricothyroid muscle.
Electromyographic recording was usually done at least one day after the operation, the animal being conscious or in light anesthetic condition by Ketaral®. The delay time from the kinetic activity of the geniohyoid muscle to the cricopharyngeal muscle and the relaxation time of the cricopharyngeal muscle and the cricothyroid muscle in the anesthetized dog were about two times as long as in the conscious condition.
The delay time and the relaxation time of the electromyogram were also longer in the dry solid swallowing than in the liquid swallowing.抄録全体を表示PDF形式でダウンロード (684K) -

ジャーナル フリーFive cases of nasal hemangioma were experienced in our department during the period, 1975-1981. The following result were obtained. a) The average age of these patients was 50 years. b) These five cases accounted for 15% of our patients with nasal tumor. This showed hemangioma occupied on important position of the nasal tumor. c) Epistaxis and progressive nasal obstruction were frequent symptomes, especially all patients complainted epistaxis. d) Surgical treatment and cryosurgery were effective and no recurrence was shown. ジャーナル フリーIn the field of otorhinolaryngology many kinds of animals, e. g. mouse, rat, guinea pig, rabbit, cat, dog and monkey have been used as experimental animals; but the chicken is not commonly used for research on the hypopharynx, larynx, trachea and esophagus. In the chicken, the larynx has sphincter action but has no phonetic function. The syrinx, which is the carina in birds, is the voice producing organ and contains two pairs of tympanic membranes which play the part of vocal folds. So birds have two larynges, the real or upper larynx and the carina or lower larynx. The advantages of the chicken for experimental studies are its docile temperament, ease of handling and low cost. In addition, since the chicks are received from 24 to 48 hours after hatching, their ages are known exactly. The chicken is suitable for experimental studies which require a large number of experimental animals of the same age and have natural phonation. In this paper, anesthesia, blood collection, anatomy of the upper and lower larynx, phonation, and analysis of the voice of the chicken are discussed as well as prior studies of phonation or voice in other species of birds.抄録全体を表示PDF形式でダウンロード (5563K) -

ジャーナル フリーA 13-year-old girl was found, on physical examination at school, to have a yellowish white osteoma-like tumor mass in the middle portion of her left nasal cavity. On x-ray examination, a tooth-like shadow was noted in her left nasal cavity. As it was considered impossible to remove the mass via the external naris, the maxillary sinus was opened through an alveolotomy and upon widening the natural opening the mass was removed. It was found to be a supernumerary canine tooth-like dens. ジャーナル フリーA study was made of recently advocated methods of treatment for sudden deafness. Not being certain of the effectiveness of any one method of treatment used alone, the author employed inhalation therapy with a 5% CO2-95% O2 mixture in varying combination with 76% urografin injected intravenously, adrenocortical hormones given in large doses by intravenous drip infusion and low molecular dextran administered intravenously by drip infusion, concomitantly with circulatory stimulants, large doses of vitamins B1, B6 and B12 and orally administered metabolic stimulants of the inner ear. Contrary to our expectations, these combinations of therapeutic regimens proved to be no better than those in conventional use, particularly in terms of cure rate. An account is given, for reference, of some cases in which the recovery of hearing could successfully be attained by changing regimen during the course of treatment.抄録全体を表示PDF形式でダウンロード (911K) -

One hour after an intravenous injection of 1 and 2g CMZ, its serum level was as high as 45.3 and 70.5μg/ml respectively, and its concentration in the aural discharge was 21.4 and 23.8μg/ml respectively. 2. One hour after an intravenous injection of 0. 5g of CMZ in infants, and 1g CMZ in adults, the serum level of CMZ was 60 and 40.5μg/ml respectively, and its concentration in the tonsil was as high as 15.2 and 9.6μg/g, respectively. 3. CMZ was administered to 38 patients with infections in the otorhinolaryngology clinics. The drug was markedly effective in 22 cases and effective in 11 cases: The effective rate was 86. 8%. The effect of CMZ on isolated bacteria was investigated bacteriologically. The drug proved to be effective against gram-positive bacteria including H. influenzae and P. mirabilis. Moreover, the MBC/MIC of CMZ was determined in isolated bacteria. The drug exerted a strong bactericidal effect at a concentration near MIC. 4. No side effect was found in any of the 38 cases. In addition, no case gave abnormal findings in clinical tests. 5. Good therapeutic results with few side effects is expected with CMZ in the treatment of infections in otorhinolaryngological clinic except for Pseudomonas infection.抄録全体を表示PDF形式でダウンロード (3530K) -

ジャーナル フリーAuthors clarified that clinical effects of cefmetazole (CMZ) against to gram positive bacterial infection were very useful and valuable, in our otorhinolaryngological region. 13 patients who were infected with gram positive bacteriae had been improved after intravenous drip infusion at dosis of 2 gr per day for 3 or 5 days. ジャーナル フリーTo improve the capacity of Killian-lamp the author innovated a head mirror by employing glass fibers as light guide. The light guide was devised by Nagashima Co. according to SL-lighting system. The merit of this improved mirror is that it lights up the visual field brighter and makes deeper region be observed better than a reflector of 100 W lamp. Accordingly the mirror contributes to the smooth performance of operation and the avoidance of side-injury. With this new type of mirror deeper region can exactly be observed at ambulatory examination or operation.
