    SPG (a β-1, 3-D-glucan from Schizophyllum commune Fries) is known as one of the immunopotentiating agent showing antineoplastic effects in experimental animals. SPG was administered intramuscularly 40 milligrams a week for three months to 30 patients with head and neck malignant tumors in combination with the usual therapy of surgery, irradiation and chemotherapy. Therapeutic effect of SPG was evaluated from the clinical standpoint. Results were as follows: ten extremely effective, one effective, seven slightly effective, ten ineffective. Neutrophil function tests were performed at pre-and post-treatmentstages. Both chemotactic activity under agarose plate and NBT reductive capability, which reflect bactericidal activity, were not affected by administration of SPG. Two patients complained of pain at the site of inyection. Other side effects were not observed.

    For vocal rehabilitation after laryngectomy, various methods have been devised, such as Serafini's or Straffieri's reconstructive laryngectomy or tracheoesophageal shunt operation. These techniques are popular in head and neck surgery, however these methods have several disadvantages for speech because aspiration is sometimes unavoidable. We have developed a surgical technique and have been applying it to laryngectomized patients. The surgical peocedures are as follows: 1. When the hyoid bone is removed with the larynx, only the upper portion of the thyropharyngeal muscle is sutured together, but the lower portion is not. The free margins of the cricopharyngeus muscle are not sutured. 2. When the hyoid bone is not excised, both sternohyoid muscles are interposed btween thyropharyngeal muscles and sutured together with them. In either cases, the mucosae of the hypopharynx is sutured horizontally. Only 40% of laryngectomized patients could speak with esophageal voice after laryngectomy with the routine method. With our technique, more than 80% of the acquired esophageal speech.

    A case is presented of a 5-year-old boy with fever and painfull swelling in the left lobe of the thyroid gland. An echogram of the patient revealed that the left lobe of the thyroid gland was diffusely swelling, in which there was the echofree area that was suspected of an abscess formation. The abscess was drained and the patient was treated with broad spectrum antibiotics for two weeks. After three months he was admitted to our hospital again with similar symptoms. He developed a mild upper respiratory infection one week before admission. An echogram revealed that the left lobe of the thyroid had diffuse swelling, and the right lobe was partially swollen. He was treated with the same therapy for two weeks and discharged. He has been in good health since discharge. The diagnostic value of the echogram for acute suppurative thyroiditis is stressed.

    A 62-year-old man visited our clinic in September, 1980, with a complaint of a small mass formation in the right auricle. He had first noticed it approximately 8 years before. On physical examination, there were two tumors in the right auricle, one was yellowish, the other was dark-brown. Under local anesthesia, the tumors were excised and a split thickness skin graft was implanted from the thigh. Pathological examination revealed two types of coexisting basal cell carcinoma, nodulo-ulcerative type and pigmented type. No sign of recurrence and metastasis has been detected one year and four months after the operation. Only five cases of auricular basal cell carcinoma have been reported in the Japanese literature.

  • Effect of ATP on experimental sludge phenomena

    By repeating anoxia on a guinea pig, pathologic intravascular red cell aggregation (sludge phenomena) being accompanied with lowering of blood flow velocity resulted in the radiating cochlea arteriole that could not be recovered by only oxygen supply. By an intraperitoneal injection of adenosine triphosphate (ATP) in this model, the recovery conditions of the radiating arteriolar blood flow were continuously observed by microscope and recorded on 16mm color film. The velocity of the blood flow was measured by the recorded 16mm color film, and the heart rate was obtained from the recorded electrocardiogram. The results are as follows: When pathologic intravascular red cell aggregation was observed in the radiating arteriole, the velocity of the blood flow was 120μm/sec, calculated blood volume: 0.3×10-3mm3/min and heart rate: 82/min. Ten minutes after ATP injection, these rates increased up to 600% (720μm/sec), 633% (1.9×10-3mm3/min) and 113% (93/min) respectively, and also these recovery rates were 90%, 90% and 78% respectively, showing a remarkable improvement. As a results, ATP should be considered to be effective on the improvement of velocity of the blood flow and blood volume of radiating arteriole in the cochlea. Pharmacological mechanism of ATP concerning with the improvement of blood flow velocity of radiating arteriole in the cochlea was discussed in this article.

  • Histopathological Changes in the Syringeal Membranes in the Chicken

    The pathogenesis of dysphonias induced by androgenic hormones is not clearly understood even now. In this experiment, studies on a same material were performed not only phoniatrically but also histopathologically at the same time in order to shed new light on the pathogenesis. 24-hour-old female chickens were injected with androgen regularly to bring about a condition of dysphonias. After checking to be sure it was present, the ordinary sounds they produced were analyzed phoniatrically, while the lateral syringeal membrane (L. S. M.) and the medial syringeal membrane (M. S. M.) were examined histopathologically. The results were as follows; 1) Not only irregular shapes and amplitudes of the waves, but also a lowering of the fundamental frequency were observed on the oscillogram, while a lowering of the central frequency and a widening of the frequency ranges were observed on the spectrumgram. 2) When viewed under a light microscope, both the L. S. M. and the M. S. M. showed that in the lamina propria mucosa (L. P. M.), collagen fibers were running irregularly with an increase in the number of capillaries. 3) Under examination by an electron microscope, irregularly running collagen fibers as well as an increase in the number of the capillaries could be observed in the L. P. M. of the L. S. M. and the M. S. M. Finally, it was concluded that the phoniatric changes in the voices of chickens were caused by the material changes in the syringeal membranes which were the vibratory ones.
